Özet

Bu çalışma Romatoid Artritte (RA) tibialis posterior tendonu (TPT) tenosinovitine bağlı olarak gelişebilecek ayak deformitelerini değerlendirmek amacıyla yapılmıştır. Bu amaçla 24 RA'li hastada bilateral TPT'nu klinik, radyolojik ve ultrasonografik olarak değerlendirilerek sonuçlar 15 kişilik sağlıklı kontrol grubuyla karşılaştırıldı. Klinik olarak arka ayak ağrısının derecesi, tek topuk yükselme testi, tibiokalkaneal açı, radyolojik olarak lateral yük verilerek çekilen ayak grafilerinde talometatarsal açı, ultrasonografik olarak da tendon devamlılığı ve kalınlığı değerlendirildi. Anormal tek topuk yükselme testi olan RA'li hastalarda TFT kalınlığı (2.14±0.65 mm) normal tek topuk yükselme testi olan RA'li hastalara (3.48±0.50 mm) ve kontrol grubuna göre 3.76±0.43) anlamlı olarak düşük bulundu (Sırasıyla p değerleri p=0.001, p<0.001). RA'li hastalarda TPT kalınlığı ile tibiokalkaneal açıyla değerlendirilen topuk valgusu arasında ve talometatarsal açıyla değerlendirilen pes planus arasında anlamlı korelasyon saptandı (r=-0.42, p<0.05; r=0.65, p<0.001). TPT kalınlığı ile RA'li hastaların hastalık süresi ve yaşı arasında korelasyon bulunmazken, TPT kalınlığı ile fonksiyonel evre arasında anlamlı korelasyon bulundu (r=-0.52, p<0.01). Bu sonuçlar romatoid tenosinovitin TPT'unda incelmeye ve disfonksiyona yol açarak topuk valgus ve pes planus deformitelerine neden olduğunu göstermektedir.

Abstract

The aim of thts study was to determine the foot deformities that could occur secondary to tenosynovitis of tibialis posterior tendon (TPT) in Rheumatoid Arthritis (RA). For this purpose bilateral TPT of 24 RA patients were assessed by clinical, radiologic and ultrasonographic methods. The findings were compared with the findings of 15 healthy people who were considered as control group. Clinically the degree of hindfood pain, single heel rise and tibiocalcaneal angle were assessed. Radiologically talometatarsal angle was measured from the lateral standing foot radiographs. In ultrasonographic evaluation the continity and thickness of TPT was measured. In RA patients with abnormal single heel rise test, the thickness of TPT (2.14+0.65 mm) was found less than the RA patients with normal single heel rise test (3.48±0.50 mm) and the control group (3.76±0.43) and the differences were statistically significant (p values were= 0.001 and <0.001 respectively). In RA patients the TPT thickness was significantly correlated with the hell valgus which was calculated from the tibiocalcaneal angle and with pes planus which was measured from the talometatarsal angle (r=-0.42, p<0.05; r=0.65, p<0.001 respectively). The age of the RA patients and the duration of the disease was not correlated with the thickness of TPT but a significant correlation was obtained between the TPT thickness and functional class of the RA patients (r=-0.52, p<0.01). These findings imply that the tenosynovitis in RA leads to thinning disfunction in TPT and as a result, deformities as heel valgus and pes planus could occur in RA foot.
