The value of Tc-99m citrate for imaging arthritic joints in rheumatoid arhtritis

Özet

Bu çalışmanın amacı , romatoid artritli hastaların değerlendirilmesinde Tc-99m Sitrat'ın tanı değerini belirlemekti.ARA tanı kriterine göre ekstremitelerinde romatoid artriti olan 28 hasta (yaş ortalaması 43.89±10.17 yıl) ve 10 normal (yaş ortalaması 41.13+9.35 yıl) kişinin izinleri alınarak çalışma grubuna alındılar. Her hastaya 555 MBq Tc-99m sitrat intravenöz olarak uygulandı. Tüm vücut ve tutulan eklemlerin spot görüntüleri gama kamera ile üçüncü saatte alındı. Hastalarda, klinik olarak 70 büyük eklemde ve sintigrafık olarak da 108 eklemde tutulum tespit edildi. Tc-99m sitrat başlıca sinovial boşlukta toplandı. İnflamatuar eklemler ve "background" olarak ta komşu yumuşak dokudan ilgi alanları alındı. Böylece, eklem/yumuşak doku (J/B) oranları hesaplandı . Ortalama J/B oranları hastalarda 1.91+0.67 iken kontrol grubunda 1.18+0.23 idi (p<0.001). Biz, Tc-99m sitrat sintigrafisinin etkilenmiş eklemleri teşhis etmede yararlı bir metod olduğu sonucuna vardık.

Abstract

The purpose of this study was to determine the diagnostic value of Tc-99m citrate in the evalua¬tion of patients with rheumatoid arthritis. Twen¬ty-eight patients (mean age: 43.89±10.17 years) with definitive rheumatoid arthritis in extremi¬ties, based on ARA diagnostic criteria and 10 normal subjects ( mean age : 41.13±9.35 years) were included in the study group after obtaining their informed consent. 555 MBq Tc-99m citrate was administrated intravenously to each pa¬tient. Whole-body and spot images of the in¬volved joints were obtained at 3 h with a gamma camera. In patients, 70 large joints clinically and 108 joints scintigraphically were involved. Tc-99m citrate accumulated mainly in the syno-vial space. Regions of interest (ROI's ) was drawn over the inflammatory joints and a sec¬ond ROI over the nearby soft tissue as a back ground. Then, joints/soft tissue (J/B) ratios were calculated. The mean J/B ratio in patient and control groups were 1.91±0.67 and 1.18±0.23, respectively (p<0.001). We conclud¬ed that Tc-99m citrate scintigraphy is a useful method in order to diagnose the affected joints.
