Özet

Cerrahi sırasında kansız saha oluşturmak amacıyla turnike kullanımını takiben ekstremitede gelişen paralizi, iyi bilinen ancak nadir bir komplikasyondur. 3 yıllık süre içinde EMG laboratuarımıza başvuran olguların 35'inde turnike paralizisi ile uyumlu elektrofizyolojik bulgular saptanmıştır. Retrospektif olarak ayrıntılı bilgi edinilebilen 12 olgunun, travmatik sinir ve/veya tendon kesişi ve kemik fraktürü nedeniyle opere edildiği ve hepsinin operasyonu sırasında turnike kullanıldığı öğrenilmiştir. Bu makalede, tipik olan 4 olgunun seri elektrofizyolojik bulguları sunulmaktadır. Sinir lezyonlarının muhtemelen yüksek turnike basıncına bağlı geliştiği düşünülmüştür. Makalede, turnike paralizisi tanısında elektrofizyolojik incelemelerin önemi vurgulanmakta, özellikle travmatik periferik sinir lezyonları olan olgulara uygulanan ekstremite cerrahisi sonrasında yapılan klinik ve elektrofizyolojik değerlendirmelerde turnike paralizisinin hatırlanması ve bu doğrultuda yönlendirilmesi gerekliliğine dikkat çekilmektedir.

Abstract

Paralysis of the limb after use of a tourniquet for establishing bloodless field during surgery is a well-recognized but rare complication. Electrophysiological findings consistent with tourniquet paralysis have been found in 35 cases referred to our EMG laboratory during 3 years period. Detailed information could be obtained retrospectively in only 12 cases and it was learned that they had underwent surgical procedures for traumatic nerve and/or tendon injuries and bone fractures, with the tourniquet use in all of them. In this paper, serial electrophysiological findings of 4 typical cases are described. Nerve lesions were considered to be pro-duced by the high tourniquet pressures. The importance of the electrophysiological studies in the diagnosis of tourniquet paralysis and the necessity to keep tourniquet paralysis in mind in clinical and electrophysiological evaluations particularly after surgical procedures for traumatic nerve lesions have been emphasised in the paper.
