Özet

Romatoid artrit (RA)'de bazı prognostik kriterler varlığında atlanto-aksiyel subluksasyon (AAS) gelişme riskinin daha fazla olduğu bildirilmektedir. Bunlar seropozitivite, uzun süreli hastalık varlığı, subkutan nodul varlığı ve uzun süreli kortikosteroid (KS) kullanım öyküsüdür. RA'deki AAS'da klinikte oksipito-servikal boyun ağrıları or-taya çıkabildiği gibi sensoriyel bozukluklar, piramidal traktusun etkilenmesine bağlı bulgular, vertebrobaziler yetmezlik bulguları da ortaya çıkabilir. Çalışmamızda 40 RA'li hastada radyografiler ve bilgisayarlı tomografi (BT) ile AAS'ları tesbit etmeyi, AAS'un prognostik kriterler, klinik ve nörolojik bulgular ile ilişkisini saptamayı amaçladık. Ayrıca somatosensoriyel uyarılmış potansiyel (SEP) çalışmasının bize klinikte yararlı olup olamayacağını araştırdık. Sonuç olarak AAS tanısında radyografiler yeterli bulunmazken, BT'nin tanıda üstünlüğü tartışılmaz idi. RF titrasyonu yüksek olan, uzun hastalık süresi olan, eroziv eklem hastalığı bulunan, subkutan nodul saptanan ve uzun süreli KS kullanım öyküsü olan hastalarda AAS meydana gelmesi anlamlı şekilde yüksek bulundu. AAS olan olgularda oksipito-servikal boyun ağrısı, sensoriyel duyu ve vertebrobaziller yetmezlik bulguları anlamlı derecede fazla idi. AAS'larda medulla spinalisin değişik bölgeleri etkilenebilir. Medulla spinalisin dorsal kolumnasının incelenmesinde SEP'in bize değerli bilgiler sağlayabileceği sonucuna varıldı.

Abstract

It is known that in rheumatoid arthritis (RA) the development of atlanto-axial subluxation (AAS) is more common in the presence of some of the prognostic criteria. These criteria are seropositivity, presence of long standing disease, presence of subcutaneus nodules and history of corticosteroid use for a long period. Although occiputo-cervical neck pain can be seen in AAS in RA, also sensory disturbances, semptoms of pyramidal tract involvement and vertebrobasillar insufficiency can be seen. In our study we aimed to diagnose AAS with plain radiographs and computed tomography (CT) and to establish the relationship between prognostic criteria of AAS and clinical and neurologic symptoms of RA in 40 patients. Also we investigated the usefullness of somatosensory evoked potentials (SEP) in clinical practice. As a result, although the usefullness of plain radiographs in the radiognosis of AAS wasn't promising, superiority of CT was definite. Occurence of AAS was meaningfully higher in patients with high rheumatoid factor positivity, long standing disease, erosive joint disease, subcutaneus nodules and history of long lasting corticosteroid treatment. In AAS cases, occiputo-cervical neck pain, sensory findings and symptoms of vertebrobasiller insufficiency were significantly higher. Different regions of medulla spinalis can be affected in AAS and as a result, the investigation of dorsal columna of medulla spinalis by SEP can give beneficial information.
