Özet

Bu çalışmaya spastisitesi olan 22 hemiplejik hasta ile 18 sağlıklı kişi alındı. Hemiplejik hasta ve kontrol grubundaki kişilerin herhangi bir antispazmolitik ajan kullanmıyor olmasına dikkat edildi. Tüm olguların alt ekstremite sinir iletim çalışması yapılarak, polinöropatisi olanlar çalışmaya alınmadı. Olguların aynı seansta elle kullanılan elektronik refleks çekiciyle Achille T refleksine ve H refleksine bakıldı. Ayrıca hastaların spastisitesi Ashworth skalasıyla değerlendirildi. Hemiplejik hastaların spastik taraflarındaki T refleksi maksimum amplitüd (p 0.05). Sonuç olarak; elle kullanılan elektronik refleks çekici aracılığıyla elde edilen T refleksinin pratikte güvenilir olarak kullanılabilecek bir test olduğu ve spastisitenin kantifikasyonunda kullanılabileceği düşünüldü.

Abstract

22 Hémiplégie patients with spasticity and 18 healthy controls were included in this study. Emphasis was put on the fact that neither the hémiplégie patients nor the healthy controls were taking any antispasmolytic agents. Nerve conduction studies in the lower extremity were performed in all cases and those found to have polyneuropathy were excluded from the study. The Achilles T and H reflexes were recorded with an electronic handheld hammer. To evaluate spasticity, Ashworth scala was performed. When the maximum value of T (p<0.01) and H reflexes (p<0.005) and the ratio of HIM amplitude (p<0.001) were compared with uninvolved extremity and with those of the controls, they were found to be significantly higher in the hémiplégie spastic extremity. When compared with the Ashworth values of the spastic side, the increase in the T reflex amplitude (p < 0.02) was found to be related with the degree of spasticity. But the H reflex amplitude and the ratio of H/M amplitude were found statistically insignificant (p>0.05). It is concluded that T reflex obtained by the hand-held electronic hammer is a practical test which can be used in quantifying the spasticity.
