Özet

Ankilozan Spondilit (AS), çoğunlukla genç erkeklerde ortaya çıkan, öncelikli olarak sakroiliak eklemleri ve omurgayı tutan, ancak tüm vücut sistemlerini etkileyebilen ilerleyici bir hastalıktır. Hastalık süreci içinde, birçok komplikasyonların yanı sıra atlanto-aksiyal subluksasyon, omurga fraktürleri, spondilodiskitler ve olasılıkla buna bağlı radikülo medüller komplikasyonlar ile de karşılaşılabilmektedir. Bu sonuncusunda, klinik görünüm Kauda Ekina (KE) sendromu biçiminde karşımıza çıkmaktadır. Bu yazıda Roma ve New York tanı kriterlerine göre (1) kesin AS tanısı konulmuş bir erkek olgudaki KE Sendromu klinik tablosu ile bu olguda gerçekleştirilmiş incelemeler ve klinik gelişim sunulmuş, bulbokavernöz refleks incelemesinin tanıda önemine değinilmiştir.

Abstract

Ankylosing Spondylitis (AS) is a progressive disease that is mostly found in young males, that affects the sacro-iliac joints and the spine especially, but that can have effects in all the body systems. During the progression of the disease, in addition to many complications, atlanto-axial subluxation, fractures in the spine, spondylodiscitis and, probably resulting from this, radiculo-medullar complications can be seen. This last complication an present as the Cauda Equina Syndrome (CES) clinically. In this article, the CES clinical picture seen in a male subject with a diagnosis of defirtite AS according to the Rome and the New York Diagnostic Criteria, the investigations performed and the clinical developlent is presented; and the importance of bulbocavernosus reflex in the diagnosis in discussed.
