Lomber apofizer artroz olgularında paraartiküler infiltrasyonlar ile fizik tedavinin karşılaştırılması

Özet
Bu çalışmaya lomber apofizer artroz tanısı konmuş 106 hasta alındı. Bu olguların 37'si lokal anestezik grubu, 34'ü lokal anestezik+kortikosteroid grubu, 35'i ise fizik tedavi grubu idi. 11 hasta ilk değerlendirmeden sonraki kontrollere gelmediğinden çalışma dışı bırakıldı. Olgular 4 hafta takip edilidi. 4 hafta sonunda global iyileşme oranı lokal anestezik grubunda %79.6, lokal anestezik+kortikosteroid grubunda %78.2, fizik tedavi grubunda ise %84.7 olarak saptandı. Her üç grup birlikte değerlendirildiğinde ise bu oran 4 hafta sonunda %79.8 idi.Ekstansiyon kısıtlılığı ve hareket ağrısı açısından fizik tedavi grubu lehine diğer iki gruba oranla istatistiksel açıdan anlamlı farklılık saptanırken (p<0.05), vizüel ağrı skalası ve 100 m. yürüme testine göre değerlendirmede gruplar arasında istatistiksel açıdan anlamlılık yoktu.Kronik bel ağrısı olan hastalarda apofizer artroz tanısı daima göz önünde tutulmalıdır. Apofizer artroz tanısı almış hastalarda ilk tedavi planının fizik tedavi ve rehabilitasyon olması gerektiği kanısına varıldı. Bu tedavinin uygulanamadığı durumlarda paraartiküler infiltrasyonların da iyi bir tedavi yöntemi olduğu benimsendi.
Abstract
106 patients diagnosed as lomber apophyseal arthrosis were included in this study. 37 of the patients were in local anestetic group, 34 were in local anestetic+corticosteroid group and 35 were in physical therapy group. 11 patients were excluded for they didn't come to follow up examinations after first assesment. We followed up the cases for four weeks. In the end of 4 weeks global improvement rate was %79.6 in lo¬cal anestetic group, %78.2 in local anestet¬ic+corticosteroid group and %84.7 in physical therapy group. When three groups were as¬sessed together, the same rate was %79.8. Physical therapy group showed statistically sig¬nificant (p<0.05) improvement in extension limi¬tation and pain in motion compared to the other groups, otherwise in visuel pain scale and 100 meters walking test there was no statistically significant difference among the three groups. The diagnosis of apophyseal arthrosis should always be considered in patients having chronic low back pain. In the end of this trial we con¬cluded that, the first therapeutic approach in pa¬tients diagnosed as apophyseal arthrosis should be physical therapy and rehabilition. An¬other good choice is paraarticular infiltration, espacially in conditions that physical therapy cannot be perfomed.
