Kafa travmalı hastalarda kognitif bozuklukların değerlendirilmesi ve günlük yaşam aktivitelerine etkileri

Özet
Kafa travmalı hastalarda fiziksel fonksiyon bozuklukları yanında, kognitif defisitler ve davranış bozukluklarının da doğru teşhis ve tedavisi gereklidir. Çalışmamızın amacı kafa travmalı hastaları kognitif fonksiyonel yönden değerlendirmek ve mental fonksiyon bozuklukları ile günlük yaşam aktiviteleri arasındaki ilişkiyi araştırmaktı. Yaş ortalaması 25.8 olan 25 hastaya kognitif değerlendirme için Mini-Men¬tal Test ve Rancho Los Amigos Skalası, fonksiyonel değerlendirme için Barthel indeksi uygulandı. Rehabilitasyon programı öncesi ve sonrası saptanan skorlar arasındaki fark anlamlıydı (p<0.05). Yaş, koma süresi, posttravmatik amnezi süresi, eğitim düzeyi ile test sonuçları arasında anlamlı ilişki saptanmadı. Kognitif skorlarla günlük yaşam aktiviteleri arasındaki anlamlı korelasyon kognitif fonksiyon bozukluklarının hastaların fonksiyonel düzeylerini önemli ölçüde etkilediğini gösterdi.
Abstract
It is necessary to diagnose and treat the cognitive defi¬cits and behavioral disorders properly besides the physi¬cal disfunctions in head injury patients. The aim of this study was to assess the cognitive functions and the re¬lationship between mental disorders and daily living activities in head injury patients. Mini Mental Test and Rancho Los Amigos Scale were applied for cognitive assessment and Barthel index was applied for functional assessment at the begining and at the end of the reha¬bilitation program in a group of 25 patients, with the mean age of 25.8 years. The difference between the score evaluated before and after rehabilitation program, was statistically significant (p<0.05). There was no signifi¬cance between the scores and age, duration of coma, posttraumatic amnesia and educational level. The posi¬tive correlation between cognitive scores and daily liv¬ing activities showed that cognitive disorders influenced the functional outcome of the patients.
