Özet

Diabetes mellitusta kas-iskelet sistemine ait hastalıklarla oldukça sık karşılaşılır. Bunlardan;nöroartropati, septik artrit, osteomiyelit ve kısıtlı eklem hareketi direkt komplikasyonları, omuz periartriti, Dupuytren kontraktürü, fleksörtenosinovit, karpâl tünel sendromu ve Forestier hastalığı (DISH) ise indirekt komplikasyonları oluşturur. Diabetes mellitusun gut, kalsiyum pirofosfat dihidrat kristal depo hastalığı, osteoartrit ve osteoporoz ile de olası ilişkisi vardır. İnsülin bağımlı diabetes mellituslu hastalarda azalmış kemik mineral yoğunluğu tartışmasız kabul edilmektedir ancak tip II-insülin bağımlı olmayan-diabetle kemik mineral yoğunluğu arasındaki ilişki hala tartışmalıdır. Bu çalışmada tip II-insülin bağımlı olmayan-diabetli, normal böbrek fonksiyonlarına sahip 44 postmenopozal kadın hastada lomber omurga (L2-L4) ve femur boynu kemik mineral yoğunluğu dual enerji X-ray absorpsiyometri (DEXA) ile ölçülerek, sonuçlar yaş-cinsiyet uyumlu 27 kişilik kontrol grubu ile karşılaştırıldı. Diabetik hastalarda her iki bölgede de kemik mineral yoğunluğu kontrol grubundan farklı bulunmadı. Ancak hastalık süresi 5 yılın üzerinde olan grupta femur kemik mineral yoğunluğunda kontrol grubuna göre düşüklük bulundu. Çalışmamızın sonuçları; diabetik hastaların hastalık süreleri ile ilişkili olarak osteoporoz için artmış risk altında olduklarını göstermektedir.

Abstract

The musculoskeletal system diseases are very frequent in diabetes mellitus. Some of these, such as neuroarthropathies, septic arthritis, osteomyelitis, lim¬ited joint mobility constitute direct complications while capsulitis of the shoulder, Dupuytren's contracture, flexor tenosynovitis, carpal tunnel syndome, Forestier's disease (DISH) constitute indirect complications. There is also possible associations of diabetes with gout, cal¬cium pyrophosphate dihydrate (CPPD) crystal deposi¬tion disease, osteoarthritis and osteoporosis. Although bone mass has fairly consistently been dem¬onstrated to be decreased in patients with insulin-de¬pendent-diabetes mellitus (WDM), the relationship be¬tween bone mass and non-insulin-dependent diabetes is still controversial. In this study, bone mineral densities (BMD) of lumbar vertebrae (L2-L4) and femoral neck ofpostmenopausal 44 women with non-insulin-dependent diabetes melli¬tus (NIDDM) and normal renal functions were mea¬sured by dual energy X-ray absorpsiometry (DEXA) and the results were compared with age-sex matched 27 con¬trol subjects. In the diabetic subjects bone mineral den¬sities of either sites did not differ from the values in control subjects. However, patients with NIDDM of more than 5 years duration had significantly lower femur BMD than the control subjects. Our results suggest that diabetic patients, related to duration of the disease are at an increased risk for os¬teoporosis.
