Detection of deep-vein thrombosis by color Doppler imaging in stroke patients

Derin ven trombozu (DVT) stroklardan sonra hastaların hemiplejik bacaklarında meydana gelebilir. Pek çok rapor DVT'nun araştırılmasında kontrast venografinin kullanımını tanımlanmıştır. Bununla birlikte strok'lu hastalarda kontrast venografi ile renkli Doppler görüntülemeyi (RDG) karşılaştıran prospektif bir çatışma rapor edilmemiştir. Bu çalışma hemipleji ve/ veya hemiparezi'li 65 hastada RDG'nin diagnostik değerini değerlendirmek için yapıldı. Stroklu hastalar venografi ve RDG ile incelendi. DVT tanısında kontrast venografi standart diagnostik metod olarak kabul edildi. DVT'nun tesbitinde kontrast venografi ile RDG arasında istatistiksel olarak anlamlı bir fark bulamadık. Venografi 21 hastada DVT'nun mevcudiyetini gösterdi. RDG 21 hastanın 18'inde pozitifti (Sensitive % 86). RDG, negatif venogram'lı 39 hastanın 37'sinde normaldi (Spesifite %96). RDG'nin sensitivitesi proksimalven trombozu için %93, baldır ven trombozu için %66 idi. DVT'nun araştırılmasında RDG'nin en az venografi kadar sensitif ve spesifik olduğu sonucuna varıldı.
Abstract
Deep-vein thrombosis (DVT) may occur in the hemiple-gic leg of patients after strokes. Many reports have described the use of contrast venography for the de¬tection of DVT. However, no prospective study com¬paring Color Doppler imaging (CDI) with contrast venography has been reported in stroke patients. This study was performed in 65 consecutive patients with hemiplegia and/or hemiparesis in order to as¬sess the diagnostic value of CDI. Contrast venogra¬phy was regarded as the standart diagnostic method for DVT diagnosis. We found no statistically signifi¬cant difference between CDI and contrast venogra¬phy in the detection of DVT. In 21 patients, venogra¬phy showed the presence of DVT, CDI was positive in 18 of these 21 patients (sensitivity of 86%). CDI was normal in 37 of the 39 patients with a negative veno-gram, a specificity of 96%. The sensitivity öf the CDI for proximal vein thrombosis was 93% and for calf vein thrombosis 66%. It is concluded that CDI is at least as sensitive and spesific as contrast venography in the detection of DVT.
