Karpal tünel sendromunda phalen ve reverse phalen testlerinin tanısal değerlerinin elektrofizyolojik olarak araştırılması

Özet

Karpal Tünel Sendromu (KTS) tanısında, bazı araştırmacılar tarafından değerleri şüpheli olarak kabul edilmekle birlikte çeşitli provakatif testler kullanılmaktadır. Bu çalışmada, Phalen testi ile daha seyrek kullanılan reverse Phalen testinin median sinir duysal iletim hızına olan etkileri araştırılmıştır. Klinik muayene ve elektrofizyolojik incelemeler sonucu KTS olarak değerlendirdiğimiz ve median sinir duysal aksiyon potansiyelleri elde edilebilen 19 hasta (7'si bilateral, 26 el) ile hastanemiz sağlık personeli arasından randomize seçilmiş 24 sağlıklı kontrol çalışma kapsamına alınmıştır. Tüm bireylere test öncesi ve l'er dakika süre ile uygulanan Phalen ve reverse Phalen testleri sonrası olmak üzere 3 kez bilek-3.parmak median sinir antidromik duyu iletimi yapıldı. Elde edilen duysal aksiyon potansiyellerinin negatif tepe noktalan çalışmaya esas olarak kabul edildi. Test öncesi ve sonrası elde edilen duyu aksiyon potansiyelleri negatif tepe noktası latansları sırasıyla KTS'lu grupta 4.6.±0.67 msn, 4.72±0.71 msn, 4.78±0.73 msn, kontrol grubunda ise 3.18±0.33 msn, 3,24±0.34 msn, 3.28±0.31 msn olarak bulundu. Her iki test sonrası tesbit edilen latans değişiklikleri hem kontrol grubunda hem de KTS'lu hasta grubunda istatistiksel olarak anlamlı olmakla (p<0.05) birlikte reverse Phalen testinden sonraki latans uzaması daha belirgin anlamlı idi. Ayrıca KTS'lu hasta grubundaki latans değişikliklerin kontrol grubundaki değişikliklere göre istatistiksel olarak daha anlamlı olduğu (p<0.05) gözlendi. Çalışma sonucunda Phalen ve reverse Phalen testlerinin median sinir duyu iletimi üzerinde etkili olduğu gösterilmiş, reverse Phalen testindeki etkilenme daha belirgin olduğundan rutin poliklinik çalışmalarında Phalen testi yerine reverse Phalen testinin uygulanmasının daha yararlı olabileceği görüşüne varılmıştır.

Abstract

Although, the diagnostic values öf some provocative tests are found questionable by some investigators, these tests are being used in the diagnosis of carpal tunnel syndome (CTS). The effects of Phalen and the less often used reverse Phalen tests on the sensory conduction ve¬locity of median nerve was investigated. Nineteen patients (26 hands included 7 bilateral CTS) who have median nerve sensory action potentials and who were diagnosed CTS in our clinic as a result of physical exam andelectrophysiologic tests and 24 ran¬domly chosen healthy hospital staff were included in this study. Pre test and after one minute Phalen and reverse Phalen tests 3. digit median nerve antidromic sensory conduc¬tion have been performed three times to the patients and controls. The peak negative points of obtained sensory action potentials were used basically in this study. Pre and post test negative peak point latency of sensory ac¬tion potentials were found 4.6±0.67 msn, 4.72±0.71 msn, 4.78±0.7msn in CTS patients and3.18±0.33 msn, 3.24±0.34 msn, 3.28±0.31 msn in controls, respectively. After two tests, although the changes of latency were statistically significant (p <0.05) in both groups, latency following reverse Phalen test was more significant. The changes of latency in CTS group were statistically more significant than controls. As a result of this study, the effects of Phalen and re¬verse Phalen tests on median nerve sensory conduction was shown and it was concluded that because the re¬verse Phalen test shown to be more effective, instead of Phalen test, reverse Phalen test can be more useful in routine outpatient clinic studies.
