Özet

Periferik fasial paralizi fasial sinirle innerve kasların akut paralizisi olup en sık görülen ve idiyopatik denen şekli Bell paralizisidir (BP). Bu çalışmanın amacı kliniğimizde takip edilen periferik fasial paralizili hastalarımızın klinik, demografik özelliklerini, lejyonların mevsimsel dağılımları ve EMG sonuçlarına göre sınıflandırmak ve değerlendirmektir. Çalışmaya kliniğimize başvuran ve yaş ortalaması 39.3 ± 16.7 yıl olan 20 kadın ve 23 erkek periferik fasial paralizili hasta dahil edildi. Tüm hastalara etiyolojik, demografik ve klinik özelliklerini içeren bir sorgulama yanında elektronöromiyografik (ENMG) incelemeler yapıldı. EMG sonuçlan hastaların klinik ve etiyolojik özelliklerine göre değerlendirildi. 28 olgu BP olarak değerlendirildi. Diğer etiyolojik nedenler sırasıyla travma, kulak hastalıkları, enfeksiyon ve neoplastik nedenler olarak bulundu. Hastalarda sağ ve sol tarafta lezyon bulma oranı farklı değildi. Klinik semptomlar olarak epifora, lagoftalmus, hiperakuzi, fasial dağılımlı uyuşukluk bunu takiben tad bozukluğu, tükrük azalması ve lakrimasyon şikayetleri mevcuttu. Fasial lezyonların mevsimsel dağılımında en fazla kış (%37.2) ve ilkbahar (%30.2) bunu takiben yaz (%25.6) ve sonbahar (%9.4) aylarında tutulum söz konusuydu. BP'li hastalarda kış aylarındaki yığılım özellikle dikkat çekiciydi. Hastaların EMG incelemelerinde 5 hastada total (%11.6), 23 hastada parsiyel (%53.5) ve 15 hastada ağır parsiyel lezyon (%34.5) tespit edildi. Lezyon tipine göre etiyolojik ve mevsimsel dağılımlar arasında anlamlı farklılık bulunmadı.

Abstract

Peripheral facial paralysis (PFP) is the acute paralysis of the muscles innervated by facial nerve and Bell's palys (BP) is the most common type named as idiopathic BP. The aim of this study was to determine the clinical, demographical properties o f the patients associated with seasonal distribution as well as to classify and assess the electromyographic (EMG) test results according to the epidemiologic properties. Twenty women and 23 men diagnosed as PFP with a mean age of 39.3 ±16.7 years were included in the study. Data about etiological, demographical and clinical properties were obtained and electroneuromyographic (ENMG) tests were performed in all patients. Twenty-eight patients were diagnosed as BP. The other etiological factors were idiopathic, trauma, ear dysfunction, infections and neoplastic diseases respectively. The left and right sides were equally involved. Epifora, lagoftalmus, hyperacusis, numbness, lacrimation and taste defects were recorded as clinical symptoms. There was a predominance of cases in winter (37.2%) and spring (30.2%) followed by summer (25.6%) and autumn (9.4%). The clusters of BP in winter was a striking feature. There was 5 patients with total, 23 patients with partial and 15 patients with severe partial lesions detected by ENMG. We couldn't find a difference between the clinical demographical and seasonal distribution of patients classified as lesion types.
