İnmeli hastalarda vizüospatial hemineglect sıklığı ve rehabilitasyon sonuçlarına etkisi

Özet
Serebrovasküler olaya bağlı hemipleji gelişimi izlenen 53 hasta vizüospatial hemineglect (HN) varlığı yönünden 4 ayrı test ile araştırılarak HN'in fonksiyonel rehabilitasyon sonuçlarına etkisi prospektif olarak incelendi. Yıldız silme test skorlarına göre sol hemiplejik olguların 12'sinde (%40), sağ hemiplejik olguların 4'ünde (%17) olmak üzere toplam 16 hastada hemineglect saptandı. HN (+) hasta grubunun hem kliniğe yatış, hem de rehabilitasyon program sonunda taburculuk dönemindeki ortalama fonksiyonel bağımsızlık ölçütü (FİM) ve APECS (Adapted Patient Evaluation Conference System) skorları anlamlı oranda düşük bulundu (p<0.05). Yatış süreleri ve rehabilitasyon etkinliği yönünden gruplar arasında fark saptanamadı. Yıldız silme, figür kopyalama ve resim çizme testleri arasında kuvvetli doğrusal ilişki saptanırken, çizgi bölme ile diğer testler arasında orta derecede doğrusal ilişki saptandı. Vizüospatial hemineglektin inme sonrası fonksiyonel rehabilitasyon sonuçları için negatif prognostik bir faktör olduğu, tüm inmeli hastalarda varlığının mutlaka test edilmesi gerektiği sonucuna varıldı.
Abstract
Fifty three consecutive patients with hemispheric stroke were assessed for visuospatial hemineglect, using 4 different test methods. The impacts of visuospatial hemineglect on the rehabilitation outcome in activities of daily living were analysed prospectively. In according to star cancellations test scores 12 of right hemi spheric (%40) and 4 of left hemispheric (%17) stroke patients were diagnosed as having visuospatial hemineglect. Mean "Functional Independence Measure" (FİM) and "Adapted Patient Evaluation Conference Sys tem" (APECS) were significantly lower in hemineglect (+) group, both at admission and discharge period. But, mean length of hospital slay and rehabilitation efficiency were not different between groups. There was highly linear correlation between star cancellation, figure copying and representational drawing test scores, but the relationship between line bisection and other test results were moderate. It was concluded that, visuospatial hemineglect is a negative prognostic factor for functional rehabilitation outcome of hemispheric stroke patients. Every stroke patient must be analysed to as certain the presence of visuospatial hemineglect.
