Özet
Bu çalışma, ürodinamik değerlendirmeyle tanı konmuş inkontinans tipini önceden saptamada stres, urge ve miks inkontinans Öyküsünün değerini araştırmak amacıyla yapılmıştır. Üriner inkontinansı olan 100 kadın hasta çalışmaya alınmıştır. Tüm hastalara inkontinans tipini belirlemek için standart sorular sorulduktan sonra ürodinamik değerlendirme yapılmıştır. Anamnezin, ürodinamik tanıyı doğru bir şekilde saptayabilirliği, ürodinamik tanı kullanılarak sensitivite, spesifite, pozitif ve negatif prediktif değerleri hesaplayarak değerlendirilmiştir. Hastaların %28'inde stres inkontinans, %20'sinde urge inkontinans, %52'sinde miks inkontinans Öyküsü mevcuttu. Stres inkontinans öyküsünün varlığı stres inkontinans için %50 sensitivite, %84 spesifite; urge inkontinans varlığı, detrüsör instabilitesi için %37 sensitivite, %74 spesifite göstermiştir. Miks inkontinans öyküsü, miks inkontinansın sensitif bir göstergesi olup (%100 sensitivite), spesifisitesi düşük (%55) bulunmuştur. Bu çalışmanın sonuçlan, daha önce yapılmış birçok çalışma ile uyumlu olarak, üriner inkontinans değerlendirilirken hasta öyküsünün tek başına yeterli olmadığını, ürodinamik incelemenin gerekliliğini desteklemiştir.
Abstract
This study was done to investigate the value of the presence of a history of stress, urge, and mixed incontinence to predict the type of incontinence diagnosed by the urodynamic evaluation. One-hundred female patients with urinary incontinence were included in the study. All patients underwent through urodynamic evaluation after answering standard questions directed to classify the type of their incontinence. The sensitivity, specificity, positive and negative predictive values of the history to detect the urodynamic diagnosis were calculated. A history of stress incontinence, urge incontinence and mixed incontinence were present in 28%, 20% and 52% of the patients, respectively. A history of stress incontinence had a sensitivity of 50% and a specificity of 84% for the diagnosis obtained by the urodynamic evaluation. A history of urge incontinence had a sensitivity of 57% to detect detrusor instability and a specificity of 74%. While, the presence of a history of mixed incontinence had a very high sensitivity (100%) to detect the urodynamic diagnosis, its specificity was quite low (55%). The results of this study, in accordance with many others, suggest that history taking is not adequate while evaluating the patients with urinary incontinence and that they should be further evaluated by urodynamic examination.
