Özet
Diabete bağlı el tutulumları olan Dupuytren hastalığı, kısıtlı eklem mobilitesi sendromu (KEMS), tetik parmak, karpal tünel sendromu, diabetik sklerodaktili ve refleks sempatik distrofi sendromu fonksiyonel özürlülüğe neden olabilen ve sık görülen diabet komplikasyonlarıdır. Bu çalışmada amacımız tip 2 diabetli hastalardaki el tutulumlarının sıklığını araştırmak ve bu tutulumlarının hasta yaşı, hastalık süresi ve mikrovasküler komplikasyonlarla olan ilişkisini belirlemektir. Bu amaçla tip 2 diabetli 63 hasta ile yaş ve cinsiyet uyumlu 30 birey kontrol grubu olarak çalışmaya alındı. Hastalarda Dupuytren hastalığı, kısıtlı eklem mobilitesi sendromu, tetik parmak, karpal tünel sendromu ve diabetik sklerodaktilinin kontrollere göre anlamlı derecede yüksek olduğu saptandı (p 0.05). Çalışmanın sonucunda tip 2 diabetli hastaların takipleri sırasında el tutulumlarının göz ardı edilmemesi ve Dupuytren hastalığı ile kısıtlı eklem mobilitesi sendromu olan hastaların mikrovasküler komplikasyonlar açısından dikkatle tetkik edilmesi gerektiği kanısına varıldı.
Abstract
Diabetic hand involvements including Dupuytren's disease, limited joint mobility syndrome, trigger finger, carpal tunnel syndrome and diabetic sclerodactyly are frequently seen diabetic complications which may cause to functional disability. The aim of this study is to evaluate the prevelance of hand involvement in type 2 diabetes mellitus and to determine the correlation between hand involvement and age, duration of the disease and microvascular complications. 63 patients with type 2 diabetes mellitus and 30 sex and age matched controls were included in the study. Dupuytren's disease, limited joint mobility syndrome, trigger finger, car pal tunnel syndrome and diabetic sclerodactyly was significantly higher in the patients than in controls (p<0.001). 60.3% of patients had at least one hand involvement. Dupuytren's disease was correlated with age and the duration of the disease and retinopathy (p<0.05). Limited joint mobility was correlated with nephropathy (p<0.05J.It'sconcludedthatdiabetic hand involvements must be taken into consideration in the evaluation of the patients with type 2 diabetes mellitus. Diabetic patients who have Dupuytren's disease and limited joint mobility syndrome should be carefully evaluated for the microvascular complications.
