New effect of calcitonin in postmenopausal osteoporosis and alterations in trace elements after calcitonin therapy

Özet

Minemi ve kemik homeostazında kalsitoninin fizyolojik rolü tam olarak anlaşılamamıştır. Normal iskelet metabolizmasında eser elementlerin rolleri araştırılmış olmasına rağmen postmenopozal osteoporozda eser elementler üzerine kalsitonin tedavisinin etkisini araştıran longitudinal çalışmalar çok nadirdir. Özellikle magnezyum (Mg) ve çinko (Zn) başta olmak üzere bazı eser elementlerin organik kemik matriks sentezi için gerekli oldukları en az otuz yıldır bilinmektedir. Mevcut çalışma 70 osteoporotik ve 20 non-osteoporotik postmenopozal kadın arasında mineral profilinin farklı olup olmadığını araştırmak ve postmenopozal osteoporotik kadınlarda 6 aylık kalsitonin tedavisinin bu eser elementler üzerindeki etkisini araştırmak için planlandı. Çalışmamızda, Mg (p 0.05). Kalsitonin tedavisi alan hastalarda tedavinin birinci ayında serum Zn seviyelerinde (p<0.001) ve tedavinin üçüncü ayında serum Cu (P<0.05) ve Mg (p<0.00l) seviyelerinde belirgin bir artış (düzelme) olduğu gözlendi. Hasta grubunun başlangıç değerleri ile kontrol grubunun değerlen arasında anlamlı farklılık mevcut iken, tedavinin üçüncü ayında anlamlı farklılık y oktu. Bu sonuçlar; l)postmenopozal osteoporozda kalsitonin tedavisinin Mg, Cu, Zn seviyelerini düzenlediğini, 2) postmenopozal osteoporozda serum kalsiyumu ve fosforu normal olduğu zaman serum Cu, Mg, Zn'nin ölçümünün daha faydalı olabileceğini, 3) osteoporozun ortaya çıkmasında dieter bileşenlerin rolünü değerlendirmek için daha fazla sayıda çalışmanın gerekli olduğunu ortaya koymaktadır.

Abstract

The physiologic role of calcitonin in mineral and bone homeostasis is not very well understood. There are very rare longitudinal studies reporting the effects of calcitonin therapy on trace minerals in postmenopausal osteoporosis despite the documented involvement of trace minerals in normal skeletal metabolism. Several trace minerals, particularly magnesium (Mg) and zinc (Zn), essential for organic bone matrix synthesis, have been known for at least three decades. The present study was designed to set if the mineral profile was different between 70 osteoporotic and 20 non-osteoporotic postmenopausal women, and to evaluate the efficacy of calcitonin therapy during six months on these trace minerals in postmenapausal osteoporotic women. In our study, the serum values of Mg (p<0.05), Cu (p<0.05) and Zn (p<0.001) were significantly lower in the patient group than those in control group. There was no statistical difference between serum values of Ca, P, ALP (p>0.05) in the patient and control group. Statistically significant improvement was observed in serum levels of Zn in the first month (p<0.001), and Mg (p<0.001) and Cu (p<0.03) in the third month in the patients receiving calcitonin therapy. While there was significant difference between patient and control groups at the baseline, there was no significant difference in the third month. These results suggest that 1) calcitonin therapy may regulate Mg, Cu, Zn levels in postmenopausal osteoporosis, 2) when serum Ca, P are normal in post menopausal osteoporosis, serum Mg, Cu, Zn are more useful for evaluation, and 3) further studies are essential to evaluate the role of dietary composition on the manifestations of osteoporosis.
