Femoral mononöropati (FMN) diğer periferik nöropatilere oranla daha az sıklıkta görülür FMN'nin en sık nedenleri femoral sinirde bası, gerilme ve iskemidir. Bu raporda 3 yıl süresince nöroelektrofizyoloji ünitemizde tanı alan II FMN vakası sunulmaktadır. FMN vakaları bu süre içinde ünitemizde elektrofizyolojik inceleme yapılan tüm vakaların %0.5'ini oluşturmaktadır. Vakalarımızda görülen en sık etyolojik faktör retroperitoneal kitlelerdir (%45 hematom, %10 neoplazi). Bir vaka hariç tüm hastalarımızda ciddi FMN bulguları saptanmış ve yoğun fizyoterapi ve rehabilitasyon programına rağmen ancak parsiyel iyileşme kaydedilmiştir. Subklinik seyreden ve hızla iyileşen vakalar tanı almadığı veya rapor edilmediği için FMN gerçek insidansı bilinmemektedir. Vakaların çoğunda spontan iyileşme olmakla birlikte FMN tanı ve tedavisinin gecikmesi durumunda ciddi morbidite ve kalıcı nörolojik hasarın gelişebileceğini vurgulamak istiyoruz.
Abstract
Femoral mononeuropathy (FMN) is seen less frequently when compared with other peripheral neuropathies. Compression, stretch, and ischemic injuries of the femoral nerve are the most common causes of FMN. This document reports 11 cases of FMN diagnosed during a 3-year period. FMN cases were 0.5% of all patients who were evaluated in our electrophysiology unit during this period. The most frequent etiologic factor was compression of the femoral nerve by retroperitoneal masses (45% hematoma, 10% neoplasm). All the patients but one suffered from severe form of FMN, and improved partially during intensive physical therapy and rehabilitation program. We believe that, as subclinical and rapidly improved cases are not diagnosed or reported, the real incidence is underestimated. Although most of the cases are self limited, delay in initiating treatment may lead to significant morbidity and irreversible neurologic dam age in severe forms.
