Özet
Bu çalışmada 41 spinalkord hasan (SKH) olan hastada nöropatik mesane tanı ve tedavisinde. ürodinamik buzlu su testinin önemi ve buzlu su uygulamasının sistometrik kapasite ve detrüsör basıncına .etkisi değerlendirildi. Rutin sistometrik incelemeyi takiben +4°C'deki nor¬mal serum fizyolojik kullanılarak hastaların mesane kapasiteleri ve detrüsör basınçları kaydedildi. Buzlu su testi cevapları hastaların mesane tiplerine göre değerlendirildi. Nörolojik olarak komplet/inkomplet, mesane tipine göre hiperrefleks /arefleks mesanesi olan hastaların oda sıcaklığındaki serum fizyolojik ve buzlu su uygulaması ile elde edilen sistometrik kapasite ve detrüsör basınç değişimleri karşılaştırıldı. Hiperrefleks mesaneli hastaların 25'inde (%83.3) buzlu su testi pozitif, arefleks mesaneli hastaların 9'unda (%82) test negatif bulundu. Buzlu su uygulaması ile hiperrefleks mesanesi olan hastalarda sistometrik kapasitede azalma ve detrüsör basıncında artış gözlendi. Uygulaması kolay olan buzlu su testinin, nöropatik mesanenin ürodinamik değerlendirmesinde tamamlayıcı olduğu ve ayrıca tanı ve tedavide yönlendirici bir katkı sağlayabileceği düşünüldü.
Abstract
In this study, 41 patients with spinal cord injury were evaluated to determine the usefulness of urodynamic ice-water test (IWT) in the diagnosis and management of neuropathic bladder and the effect of ice-water ap¬plication on the cystometric capacity and detrusorpres¬sure. After a routine cystometric investigation, bladder ca¬pacity and detrusor pressure of patients were measured using normal saline at +4 °C. IWT results were evalu¬ated according to bladder types of patients. The cystometric capacity and detrusor pressure variations of patients obtained with the application of normal sa¬line at room tempature and ice-water were compared according to complete-incomplete neurological status and hyper-reflexic-areflexic bladder type. IWT was posi¬tive in 25 (%82) patients with hiper-reflexic bladder and negative in 9 (%83.3) patients with areflexic bladder. Decrease in cystometric capacity and increase in de¬trusor pressure were observed in patients with hyper-reflexic bladder. Because it is simple to perform, IWT is a useful comple¬ment to urodynamic examinations in evaluation of neu¬ropathic bladder and it is suggested that IWT may pro¬vide supplementary information to direct differential diagnosis and management.
