The effects of calcitonin in the treatment of reflex sympathetic dystrophy

Özet
Refleks Sempatik Distrofi (RSD) yanıcı ağrı, şişlik, trofik cilt değişikliklerinden eklem kontraktürlerine ve kas atrofısine kadar değişik spektrumda bulgu verebilen bir semptomlar kompleksidir. Tedavisinde çeşitli fizik tedavi modaliteleri, egzersizler, sempatik blokaj, lokal ve sistemik kortikosteroidler, vasodilator ilaçlar ve kalsitonin gibi değişik metodlar önerilmektedir. Bu çalışmada, RSD tanısı alan hastalarda kalsitonin tedavisinin etkinliğinin görsel ağrı skoru (VAS) , deri ısısı ölçümleri, kavrama kuvveti ve kantitatif sintigrafık ölçümlerle değerlendirilmesi planlanmıştır. Bu amaçla 16 posttravmatik RSD hastası çalışmaya alınmıştır. Sonuçta, hastaların görsel ağrı skorları, kavrama kuvvetleri ve deri ısısı ölçümlerinde istatistiksel olarak anlamlı düzelme tesbit edilmiştir. Ayrıca hastaların 13'ünde sintigrafik tutulumda belirgin azalma gözlenmiştir. Sonuç olarak .kalsitonin tedavisinin RSD'de etkin bir tedavi olarak seçilebileceği düşünülmüştür.
Abstract
Reflex Sympathetic Dystrophy (RSD) is a symptom complex ranging from burning pain, swelling, and trophic skin changes of the arm or leg to contractures and muscle atrophy. A number of therapeutic ap¬proaches have been advocated for the treatment of RSD, including exercises, various physical therapeu¬tic techniques, sympathetic blockade, local and sys¬temic corticosteroids, vasodilator drugs and calcito-nin. In this study we aimed to evaluate the effect of calcitonin treatment in RSD by visual analogue scale (VAS) pain score, skin temperature, grip strength and quantitative scintigraphic evaluation. 16 patients suf¬fering from posttraumatic RSD were included in the study. We found significant improvement of VAS pain score, grip strength and skin temperature of the af¬fected extremities. Also mean tracer uptake on scin-tigraphy decreased significantly in 13 out of 16 pa¬tients. In conclusion we can suggest that calcitonin may be an effective treatment in RSD patients.
