Relationship between patterns of migration of the femoral head and rate of progression of osteoarthritis of the hip

Osteoartrit kalça eklemi ağrılarının önemli bir nedeni ve total kalça protezinin başta gelen indikasyon-lanndandır. Kalça eklemi osteoartritinde femur başının değişik migrasyon patemleri tanımlanmıştır ancak bunların hastalığın seyrine olan etkisi üzerinde bir görüş birliğine varılamamıştır. Biz, bu çalışmada, 1984-1985 yılları arasında total kalça protezi yapılan 41 hastada, öpere edilmeyen kontralateral kalça ekleminde, retrospektif olarak femur başı migrasyon paterni ile eklem aralığı daralma hızı arasındaki ilişkiyi araştırdık. Postoperatif her yıl çekilen ön-arka pelvis grafilerinde eklem aralığı mesafesi ölçüldü ve eklem aralığı daralma hızı hesaplandı. Ortalama takip süresi 103.4 ay idi. Tüm grubun medyan eklem aralığı daralma hızı 0.10 mml yıl bulundu. Femur başı migrasyon paterni, süpero-lat-eral, süpero-medial ve medial olarak sınıflandırıldı. Çalışmada elde edilen bulgulardan, femur başı migrasyon paterni ile eklem aralığı daralma hızı arasında bir ilişki olmadığı sonucu elde edildi. Ayrıca, cinsiyet, vücut kütle indeksi ve takipte kontralateral kalça protezi operasyonu riski de gruplar arasında benzerdi.
Abstract
Osteoarthritis is a major cause of hip pain and dis¬ability and the most frequent indication for total hip arthroplasty. Different patterns of femoral head mi¬gration have been described but the effects of these patterns on the progression of the disease and out¬come remain controversial. In this study, we retro¬spectively investigated the relationship between the patterns of femoral head migration and rate of joint space narrowing in the contralateral non-operated hips of 41 patients who underwent total hip arthroplasty for primary OA in 1984-1985. Joint space widths were quantified on anteroposterior pelvis radiographs performed annually since the operation, utilising dial calipers. Mean duration of follow-up since surgery was 103.4 months. The median rate of joint space narrowing was 0.10 mm/year for the entire group. The patterns were classified as supero-lateral, supero-medial and medial. Our findings suggested no association between the patterns of the femoral head migration and rate of joint space narrowing. Similarly, gender distribution, BMI and contralateral hip re¬placements during the follow-up were not different among groups.
