Özet
Pulmoner tutulum Romatoid Artrit'in (RA) iyi bilinen eklem dışı tutulumudur ve bu ilişkinin tanımlanmasındaj&jjMfâçıren değişik formları bildirilmiştir. Bu prosp&fjfift^çalışma RA'lı hastalarda yüksek resolüsyonlu bilgisayarlı tomografi (HRCT) ile kanıtlanmış interstisyel akciğer tutulumunu, klinik bulguların, akciğer fonksiyonlarının ve grafilerinin lojistik regresyon analizi ile belirlemek amacıyla planlanmıştır. Bu amaçla 34 sigara kullanmayan bayan hasta çalışmaya alınmıştır. Hastaların yaşı, hastalık süreleri, Ritchie eklem skorları, Lee fonksiyonel indeksleri, VAS ağrı skorları ve laboratuar sonuçlan belirlenmiştir. 34 hastanın 29'unda hem HRCT hem de pulmoner fonksiyon testlerinin yapılması mümkün olmuştur. Hastaların 8'inde (%27) HRCT ile interstisyel akciğer hastalığı bulunmuştur. Yapılan istatistiksel analizde karbon monoksit difüzyon kapasitesi (DLCO) (p=0.0216), total metotreksat dozu (p=0.0487) ve akciğer grafilerinin (p = 0.0228) interstisyel akciğer hastalığını belirlemede etkisi olduğu tesbit edilmiştir. Sonuç olarak DLCO değerleri, total metotreksat dozu ve akciğer grafileri RA'li hastalarda akciğer tutulumu riskini belirlemede yardımcı olabilecek parametrelerdir ve risk altındaki hastalara HRCT gibi ileri tetkikler istenerek tanı konabilir.
Abstract
Pulmonary involvement is a well known extra-articu¬lar manifestation of Rheumatoid Arthritis and since the first description of this association a variety of pul¬monary disorders have been reported in these patients. The aim of this prospective study was to predict high resolution computed tomography (HRCT) proven In¬terstitial Lung Diseae (ILD) in patients with RA by means of logistic regression analysis of clinical data, lung function and chest roentgenogram. 34 female, nonsmoking patients with RA were investigated. Each patient's age,duration of disease, Ritchie articular in¬dex, Lee functional index and medication as well as laboratory studies were recorded. Ten centimeters vi¬sual analogue scale (VAS) was used to evaluate pain. Among 34 patients, 29 could have both HRCT and pulmonary function analyses. Eight (27%) of the patients were found to have ILD with HRCT. The patients' data that had impact on ILD were carbon monoxide diffusing capacity (DLCO) (p=0.0216), total methotrexate dose (p=0.0487) and chest roentgenograms (p=0.0228). Using logistic re¬gression analysis, we predict ILD with a sensitivity of 75% and a specifity of 100%. The efficiency of predic¬tion was 93%. In conclusion.prediction of lung involvement, based on chest radiography, total MTX dose and DLCO value may help to detect patients at risk and HRCT may be reserved for those RA patients.
