Özet
Ankilozan Spondilit ile tiroid hastalıkları arasındaki ilişki konusunda literatürde çok az yayın mevcuttur. Burada ankilozan sponditit (AS) tanısı ile izlenen ve kronik tiroidit gelişen bir hasta sunulmakta, klinik seyri ve uygulanan tedavi tartışılmaktadır. 24 yaşında, HLA-B27 pozitif erkek hasta, hastalık aktivitesinin klinik ve serolojik olarak şiddetlenmesi üzerine hastaneye yatırıldı. Hastada poliartrit, ateş, taşikardi ve genel durumunda düşkünlük mevcuttu. İnfeksiyon odağı saptanmadı. Yapılan tetkiklerinde serum T3 ve T4 seviyesi yüksek, TSH seviyesi düşüktü. 4. saatteki iyot 131 uptake'i yüksek, anti-tiroid peroksidaz antikor negatif, anti tiroglobülin antikor yüksek bulundu. Tiroid sintigrafisinde Hashimato tiroiditini düşündüren tiroid bezi tesbit edildi. Steroid, propisil ve propranolol başlanan hastanın tedaviye iyi yanıt vermesi üzerine 2 ay sonra tedavisi sonlandırıldı.
Abstract
There are a few reports about the association of ankylosing spondylitis (AS) and chronic thyroiditis in the, literature. Here we report a patient suffering from AS associated with chronic thyroiditis, and discuss the clinical progress and therapy of this condition. A 24 year-old male patient with HLA-B27 positivity was hospitalized because of the deterioration of his clinical and serological activity parameters. He had polyarthritis, fever, tachycardia and general malaise without the presence of an infectious focus. Serum T3, T4, iodine uptake, antithyroglobulin antibody were found to be high with low thyroid stimulating hormone, and antithyroid peroxidase antibody was negative. The thyroid scintigraphy of the patient revealed a thyroid gland typical for Hashimato thyroiditis. The patient was treated with corticosteroids, propycil and propranolol. He responded to the therapy and the medication was stopped after 2 months.
