Medikal tedaviye yanıt alınamayan lomber faset sendromlu olgularda faset eklem enjeksiyonu

Özet
Klinik bulgular ve bilgisayarlı tomografi ile faset sendromu tanısı konulan 27 olgu çalışmaya alındı. Tüm olgulara 2 hafta medikal tedavi uygulandı ve 2 hafta sonunda yapılan değerlendirmede olgular medikal tedaviden yarar gören ve görmeyenler olmak üzere iki gruba ayrıldılar, Medikal tedaviye yanıt vermeyen 15 lomber faset sendromlu hastanın faset eklemi içine kortikosteroid ve lokal anestezik karışımı uygulandı. Faset eklem enjeksiyonları fluroskopi altında dejenere segment ve bir üzeri segment olmak üzere iki seviyede yapıldı ve metil prednisolon 10 mg (0.5 ml) ve bupivakain 10 mg (2 ml) enjekte edildi. Tüm hastalar tedavi öncesi, tedaviden 1 hafta, 1 ay ve 3 ay sonrası Visual Analog Skala, Modifiye Schoeber testi ve Roland Morris Skalası ile değerlendirildi ve sonuçlar kaydedildi. Biri dışında tüm hastalar faset eklem enjeksiyonundan fayda gördüler. Sonuç olarak hasta seçimi iyi yapıldığı takdirde, lomber faset sendromunda faset eklem enjeksiyonunun diagnostik ve terapötik olarak yararlı olabileceği kanısına vardık.
Abstract
Twenty seven patients who had been diagnosed as facet syndrome with clinical signs and computed tomography were included in the study. All patients received medical therapy for two weeks and patients allocated into two groups as benefit from medical therapy or not. Local anesthetic injections into facet joints were applied to 15 patients unresponsive to medical therapy. Facet joint injections were performed under fluoroscopy in two levels as degenerated segment and upper of this segment and metil prednisolone 10 mg (0.5 mi) and bupivacaine 10 mg 82 mi) were injected to each segment. All patients were controlled before therapy and 1 week, 1 month and 3 months later of therapy and evaluated as Visual Analogue Scale, Modify Schoeber test and Roland and Morris Scale and results were recorded. All of patients except one have been improved by facet joint injections. we concluded that lumbar facet joint injection could be a useful diagnostic and therapeutic procedure in the lumbar facet syndrome if patient se lection made carefully.
