Özet
Serebral palsy (SP) immatür beynin progresif olmayan hasarına bağlı gelişen hareket ve postür bozukluğudur. SP'li hastalarda karşılaşılan sorunlardan bir tanesi de kemik mineral yoğunluğundaki (KMY) azalmadır. Bu çalışmada SP'de KM Y'nü etkileyecek faktörler araştırıldı. Çalışma grubu SP tanısı almış 5 ile 12 yağları arasında 38 hasta ve 30 sağlıklı kontrolden oluşturuldu. KMY düzeylerini değerlendirmek için L2-4 KMY değerleri ölçüldü. Serumda kalsiyum (Ca), fosfor, alkalen fosfataz seviyelerine ve spot idrarda kalsiyum kreatinin oranlarına bakıldı.KMY'nu etkileyecek faktörlerden anne sütü alma süresi,alınan günlük Ca miktarı, toplam güneş banyosu alma süresi, antikonvülsif ilaç kullanımı,geçirilen operasyonlar sorgulandı; fonksiyonel durum ve tutulum tipi kaydedildi. Fonksiyonel durum ambulatuar ve nonambulatuar olarak belirlendi. KMY değerleri ile günlük kalsiyum alımı ve ambulatuar olma arasında korelasyon bulundu (sırasıyla r: 0.4p< 0.05, r: 0.5, p< 0,01). Ambulatuar ve nonambulatuar grup KMY'leri arasında anlamlı fark saptandı (p< 0.05). Bu bulgular KMY'yi en fazla etkileyen faktörün vücut ağırlığın m taşındığı ambulasyon olduğunu düşündürmektedir.
Abstract
Cerebral palsy (CP) is a disorder of movement and posture that results from a non-progressive lesion of the immature brain. One of the problems encountered in patients with CP is the decrease in bone mineral density (BMD). In this study the factors effecting BMD in CP was investigated. The study group comprised 38 patients with the diagnosis of CP and 30 healthy controls. Ages of the patients and the controls were between 5 and 12 years old. BMD values were evaluated by L2-4 BMD measurement. Calcium, phosphorous and alkaline phosphatase levels in serum and calcium creatinine ratio in spot urine were assessed. Among the factors effecting BMD; breast feeding period in infancy, daily calcium intake, sun light exposure lime, history of anticonvulsive therapy and history of surgical procedures were investigated; functional stats and involvement type were recorded. Functional statuses of the patients were determined as ambulatory and non-ambulatory. BMD values were correlated significantly with daily calcium intake and being ambulatory (r:0.4, p< 0.05; r:0.5,p< 0.01 respectively). BMD values of ambulatory and non-ambulatory groups were significantly different(p<0.05). These findings suggest that the most effecting factor in BMD values is the ambulation where weight of the body is beared.
