Miyofasial ağrı sendromunda ultrason ve enjeksiyon tedavisinin etkinliklerinin karşılaştırılması

Özet

Miyofasial ağrı sendromu (MAS) tetik noktalarla karakterize bölgesel bir kas ağrısı sendromudur. MAS'nun tedavisi hem musküler tetik noktaların tedavisini, hem de yeniden tetik nokta oluşumunu önlemek için tüm devam ettirici faktörlerin azaltılmasını içerir. Masaj ve ultrason gibi tedavi teknikleri tetik nokta inaktivasyonunda noninvaziv mekanik bozulma sağlarlar. Lokal anestezik, kortikosteroid, şalin, alkol veya botulinum toksin ile tetik nokta enjeksiyonu ise tetik noktanın kimyasal veya mekanik baskılanmasını sağlar. Bu çalışmanın amacı doku kompliansı ve basınç ağrı eşiği gibi objektif parametreler kullanarak MAS tedavisinde kullanılan ultrason ve lokal enjeksiyon tedavisi etkinliklerini değerlendirmek ve karşılaştırmaktır. MAS tanısı almış 40 kadın hasta çalışmaya alındı. Hastalar her birinde 20 hasta olacak şekilde randomize olarak iki gruba ayrıldılar. Birinci grup 10 seans ultrason tedavisi (1.5 Watt/6 dak.), ikinci grup ise bir defalık lokal anestezik (1 ml %2 Prilocine: Citanest) enjeksiyonu ile tedavi edildiler. Vizuel Analog Skala (VAS), doku kompliansı ve basınç ağrı eşiği değerleri tedavinin başlangıcında ölçüldü. İkinci değerlendirme ultrason grubunda on seans sonunda, enjeksiyon grubunda ise enjeksiyonu izleyen ikinci hafta sonunda yapıldı. Ölçümlerde Fischer'in doku komplians metresi kullanıldı. Her iki grubun ilk ve ikinci değerlerini karşılaştırdığımızda VAS'da belirgin azalma ve doku kompliansı ve basınç ağrı eşiği değerlerinde belirgin artış tespit ettik (p 0.05). Sonuç olarak bu çalışmada her iki tedavi modalitesinin etkinliği objektif parametreler kullanılarak belirlenmiş ve bir modalitenin diğerine üstünlüğü tespit edilememiştir.

Abstract

Myofascial pain syndrome (MPS) is a regional muscle pain disorder accompanied by trigger points. Management of the MPS including both treating the muscular trigger points and reducing all contributing factors to prevent the trigger points from redeveloping. Massage and ultrasound provide noninvasive mechanical disruption to inactivate the trigger point. Trigger point injection by using local anesthetic, corticosteroid, saline, alcohol or botulinum toxin leads to chemical or mechanical supression of trigger point. The purpose of this study is to evaluate and compare the efficacy of ultra sound and local anesthetic injection which are used for MPS treatment by using objective parameters such as tissue compliance and pain pressure threshold. 40 female patients with the diagnosis of MPS were included in this study. Patients were randomly selected to form two groups, each group consistedojf'20 patients. 10 sessions of ultrasound therapy was applied to the first group (1.5 Watt/5 min.) and the second group was treated by one injection of local anesthetic (1ml %2 prilocine: Citanest). Visual analog scale (VAS), tissue compliance and pain pressure threshold were measured at the beginning of the therapy. Second evaluation was performed at the end of 10 sessions on the ultrasound group and after two weeks for the injection group. Fischer's tissue compliancemeter is used for measurements. We detected a significant decrease in VAS and an in crease in tissue compliance and pain pressure thresh old values between the first and the second evaluation in both groups (p<0,05). When we compared change in the parameters of the groups, we didn't find any statistical significance (p>0,05). As a result, in this study the effectiveness of both therapy modalities were detected by using objective parameters and the superiority of the one modality to the other was not evident.
