Spondylodiscitis in ankylosing spondylitis: A case report

Özet

Spondilodiskit, Andersson lezyonu olarak bilinen bir destruktif lezyondur ve seronegatif spondiloartropatilerin prototipi olan Ankilozan Spondilit(AS)te nadiren görülen fakat iyi tanımlanabilen bir komplikasyondur. 7 yıllık AS hikayesi olan 32 yaşındaki erkek hasta bölümümüze 10 aydır mevcut olan hareketle kötüleşen ve istirahatle rahatlayan, bacaklara yayılımı olmayan, artan omurga ağrısı şikayeti İle başvurdu. Daha önceki bir travma hikayesi yoktu. Son üç yıldır Sülfasalazin ve İndometasin tedavisi altındaydı. Hastanın eritrosit sedimentasyon hızı(ESH) 7 mm/h, C reaktif proteini(CRP) 1.71 mg/dL normal sınırlarda idi. HLA B 27 positif idi. X-rayleri bilateral ciddi sakroiliiti (grade 3-4), sindesmofitleri, torakolumbar vertebralarda scmorl nodüllerini ve T 9-10 T 10-11 ve L 5 -S 1 seviyelerinde tip l olarak tanımlanan vertebral end-platelerin kartilaginöz kısmını tutan destruktif lezyonları göstermekteydi. Magnetik rezonans görüntülemesinde(MRG) T 9-10 T 10-11 L 5 -S 1 spondilodiskit ve L 5 -S 1 faseit görülmekteydi. Birçok yazara göre, Spondilodiskit 10 yılı aşkın bir süredir AS'i olan hastalarda ankiloze bir omurga üzerine bilinen veya şüpheli bir travma sonrası ortaya çıkan geç bir komplikasyondur ve bazı çalışmalarda spondilodiskiti mevcut olan hastalarda servikal omurga tutulumu da olan ciddi AS vardır. Diskovertebral destruksiyon çoğu vakada olduğu gibi birçok seviyede olabilir ve daha sıklıkla torasik ve lumbar omurgadadır. Bazen omurganın X-rayleri lezyonları açığa vurmak ve tespit etmek için ek grafiler, komputerize tomografi(CT), iskelet sintigrafisi, ve/veya MRG lere ihtiyaç duyulur. Bundan dolayı artan omurga ağrısının varlığı doktoru bir Spondilodiskit için uyarmalıdır.

Abstract

Spondylodiscilis, a destructive lesion also called the Andersson lesion is an uncommon but well recognized complication of Ankylosing Spondylitis (AS) which is a prototype of the seronegative spondyloarthropathies. A 32 year old man with a 7 years history of AS admitted to our department complaining of increasing pain in his spine without radiating into legs during the last 10 months which became worse by movements and relieved by rest. There was not any history of preceding trauma. He was under sulphasalazin and indomethacin therapy in the last three years. His erytrocyte sedimentation rate (ESR) was 7 mm/h, C reactive protein (CRP) was 1.71 mg/dl which is nor mal. HLA B27 was positive. X-rays showed severe bi lateral sacroiliitis (grade 3-4), syndesmophyts, schmorl nodules at thoracolumbar spine and destructive lesions involve the cartilaginous part of the vertebral end-plate at the levels of T<sub>9-10</sub>, T<sub>10-11</sub> and L<sub>5</sub>-S<sub>1</sub> which were characterized as type 1. Magnetic Resonance Imaging (MRI) showed T<sub>9-10</sub>, T<sub>10-11</sub>-L<sub>5</sub> -S<sub>1</sub> spondylodiscitis and L<sub>5</sub> -S<sub>1</sub> faseitis. According to most of the authors, sponylodiscitis is a late complication with more than 10 years duration of AS following a known or suspected trauma upon an ankylosed spine and in some studies patients with spondylodiscitis had severe AS with evidence of cervical spine involvement. Discovertebral destruction may occur at multiple levels as suggested in most cases and is more common in thoracic and lumbar spine. Sometimes X-rays of the spine do not reveal the lesions and additional X-rays, CT, skeletal scintigraphy, and/or MRI are needed to detect. That's why the presence of increased spinal pain should alert the physician to the possibility of spondylodiscitis.
