Comparison of the effects of alendronate and hormone replacement treatment in patients with postmenopausal osteoporosis and their effects on serum IL-6 levels

Bu çalışmada; hormon replasman tedavisi HRT ve ulendronat tedavisinin postmenapozal OP'lu hastalardaki etkinlikleri ile kemik mineral yoğunluğu, biyokimyasal belirleyiciler ve sitokinlerden IL-6 üzerine etkilerini karşılaştırmak amacıyla, Erciyes Üniversitesi Tıp Fakültesi FTR polikliniğine başvuran 43 postmenapozal OP'lu hasta alındı. Çalışmaya alınan postmenapozal OP'lu 22 olgudan oluşan L gruba HRT (0.625 mg/gün oral konjuge östrojen ve 2.5 mg/gün oral medroksiprogestron asetat) + 1000 mg/gün oral kalsiyum ve 21 olgudan oluşan II. Gruba oral alendronat 10 mg/gün + 1000 mg/gün oral kalsiyum tedavisi başlandı. Tedavi öncesi ve 6 aylık tedavi sonrası; lomber vertebra ve femur boynu kemik mineral yoğunluğu, biyokimyasal belirleyicilerden serum osteokalsin ve idrar hidroksiprolin ve sitokinlerden serum IL-6 seviyelerinin ölçümü yapıldı. Sonuç olarak, HRT ve alendronat tedavisinin postmenapozal OP'lu hastalarda kemik mineral yoğunluğu ve biyokimyasal belirleyiciler üzerine etkilen bakımından gruplar arasında fark olmadığı; fakat aynı hastalarda serum IL-6 seviyesindeki azalmanın HRT grubunda alendronat grubuna göre anlamlı şekilde daha fazla olduğu gözlendi.
Abstract
In order to compare the efficacy of hormone replacement treatment (HRT) and alendronate treatment in the patients with postmenopausal OP on bone mineral density, biochemical markers and IL-6, 43 women with postmenopausal OP admitted to Erciyes University, Medical Faculty Department of Physical Medicine and Rehabilitation were included in this study. The first group included 22 women with postmeno pausal OP were given HRT (0.625 mg/day oral conjugated estrogens + 2.5 mg/day oral medroxyprogesterone acetate + 1000 mg/day oral calcium) and the second group included 21 women postmenopausal OP were put on alendronate treatment (10 mg/day oral alendronale + l000mg/dayoral calcium). Before and at 6th months of the treatment, BMD of lumbar spine and the femoral neck, serum osteocalcin and urinary hydroxyprolin as biochemical markers and serum IL-6 as cytokines were measured. As a result, it was observed that there was no difference between the effects of HRT and alendronate treatments on BMD and biochemical markers in the patients with postmenopausal ÖP, but the reduction of serum IL-6 levels in the HRT group was significantly more than that in alendronate group.
