Özet

İnmeli hastaların motor gelişimlerini Ölçmede ve rehabilitasyon sonuçlarını değerlendirmede motor değerlendirme skalaları önemli bir yer tutar. Modifiye motor değerlendirme skalası (MMDS) bu skalalardan biri olmakla birlikte, klinikteki kullanımı yeterince yaygın hale gelmemiştir. Bu çalışmada amacımız; 1) Hemiplejik hastalarda motor iyileşmeyi göstermede MMDS'nın uygunluğunu araştırmak, 2) Fonksiyonel Bağımsızlık Ölçümü (FBÖ) ile karşılaştırarak fonksiyonel sonuçlarla olan korelasyonunu saptamak, ve 3) Rehabilitasyon sonuçlarını önceden belirleyici bir ölçüt olup olmadığını ortaya koymaktır. Çalışmamıza prospektif olarak ardışık 132 hemiplejik hasta alınmıştır. Tüm hastalara yatış ve taburculukta MMDS, Brunnstrom evrelemesi ve FBÖ uygulanarak motor ve fonksiyonel skorların birbiri ile korelasyonu araştırılmıştır. Sonuçlar incelendiğinde, hastaların önemli ölçüde motor ve fonksiyonel gelişme gösterdiği saptanmıştır (p<0.0001). MMDS'nin toplam skorlarının , taburculuktaki toplam FBÖ skorları ile iyi derecede korele olduğu bulunmuştur (r=0.737). FBÖ alt gruplarından mobilizasyon, MMDS ile en yüksek korelasyonu göstermiştir (r= 0.762). MMDS'nin yatıştaki toplam skorlarının taburculuktaki toplam MMDS ve FBÖ skorları ile de korele olduğu saptanmıştır (sırasıyla r=0.886, p<0.0001 ve r=0.675, p<0.001). Sonuç olarak, MMDS'nin hemiplejik hastalardaki motor iyileşmeyi günlük yaşam aktiviteleri içinde sorgulayabilen, uygulaması kolay, az zaman alan ve rehabilitasyon sonuçlarını önceden belirlemede kullanılabilecek, FBÖ ile yakın korelasyon gösteren bir yöntem olduğu kanısına varılmıştır.

Abstract

Motor assessment scalas are widely used for predicting the motor functions and stroke outcomes in hemiplegic patients. Modified motor assessment scala (MMAS) constitutes one of the motor assessment scala, however it is not widely accepted for clinical practice. In the present study, we aimed; 1) to investigate the validity of the MMAS in prediction of the motor improvement in patients with stroke 2) to compare MMAS with the "functional Independence Measurement" (FİM) in order to detect the correlation with the functional outcomes and, 3) to clarify whether the MMAS has capable of predicting the stroke outcomes. One-hundred thirty two consecutive hemiplegic patients were enrolled in the study prospectively. Brunnstrom scala, MMAS and FİM were tested to all patients at the hospitalization and before discharge, and then the results were compared with each other to find if they were correlated or not. The results showed that all patients were showed significant motor and functional improvement (p<0.0001). It was also observed that, the total scores of the MMAS were well correlated with the total FİM scores (r=0.737). From the subgroups of FIM, the mobilization showed the best correlation with the MMAS (r=0.762). The total scores of MMDS at admission were also found to be well correlated with the total scores of MMDS and FİM during discharge (r=0.886, p<0.0001 and r=0.675, p<0.001 respectively). In conclusion, the results of the present study showed that, the scores of MMAS were well-correlated with FİM scores, takes less time to administer and can interrogate the motor assessment during the daily activities and can be used for predicting the stroke outcomes.
