Postmenopozal osteoporozlu hastalarda farklı tedavilerin karşılaştırılması

Özet
Postmenapozal osteoporoz tedavisinde hormon replasman tedavisine alternatif olarak bisfosfonatlar, kalsitonin ve aktif vitamin D düşünülebilir. Bu çalışmanın amacı, postmenapozal osteoporozis tedavisinde bu tedavilerin etkinliklerini karşılaştırmaktır. Postmenapozal osteoporozis saptanan 90 hasta çalışmaya alındı. Hastalar üç farklı tedaviden birini alacak şekilde gruplara ayrıldı: (l)Günde 100 TU intranasal salmon kalsitonini, 500 mg kalsiyumla birlikte verildi, (2)Etidronat, iki hafta süreyle günde 400 mg ve bunu izleyen 10 hafta 500 mg/gün kalsiyum verildi. Bu tedavi şeması 4 kez tekrarlandı, (3)Günde 0.50 mg kalsitriol 500 mg kalsiyumla birlikte verildi. Tedaviler bir yıl sürdürülüp, tedavi bitiminde İncelemeler tekrarlandı. Kemik mineral yoğunluğu (KMY) ile yaş (r= 37, p<0.05) ve menapoz süresi (r=-0.30, p<0.05) arasında belirgin bir negatif korelasyon tesbit edildi. Tedaviden Önce ve sonra elde edilen biyokimyasal parametreler arasında anlamlı bir fark bulunmadı. Vertebral KMY'nun bütün tedavi gruplarında anlamlı olarak arttığı gözlendi (p<0.02) ve artış oranları bütün gruplarda benzerdi. Sonuç olarak kalsitonin, etidronat ve kalsitriolun postmenapozal osteoporozisli hastalarda kemik mineral yoğunluğunu benzer şekilde arttırdığı görüldü.
Abstract
In postmenapousal osteoporosis estrogens remain the first line therapy, with calcitriot, calcitonin, or bisphosphonates as the alternative. The aim of this study is to compare these alternative therapeutic regimens in the treatment of postmenopausal osteoporosis. Ninety patients were studied. The patients were randomized to receive one of the three different therapeutic regimens: (1)Intranasal salmon calcitonin was given as 100 IU/d with calcium 500 mg/d, (2)Etidronate was given as 400 mg/d for 2 weeks, followed by calcium 500 mg/d alone fur 10 weeks. This cycle w as repeated four times, (3)Calcilriol was given as 0.5 mg/d with calcium 500 mg/d. All the treatments were given for one year and investigations were repeated at the end of treatment. A significant negative correlation was found between bone mineral density (BMD) and aging (r=-0.37,p<0.05) and the duration of menopause (r=-0.30, p<0.05). No significant difference was found in biochemical parameters obtained before and after treatment. BMD of the lumbar spine increased significantly in all therapy groups (p<0.02).Rates of increment in BMD were similar in all groups. In conclusion, calcitonin, elidronate, and calcitriol significantly increased bone mineral density in patients with postmenapousal osteoporosis.
