Akut koroner sendromlu bireylerde C-reaktif protein (CRP) ve homosistein düzeylerinin çok damar hastalığı ile ilişkisi

Özet

Amaç: Artmış homosistein ve CRP düzeyinin koroner ateroskleroz için sadece bağımsız bir risk faktörü olmadığı, ay­nı zamanda koroner ateroskleroz miktarı ve yaygınlığı ile de ilşkisinin olduğunu destekleyen veriler mevcuttur. Bu ça­lışmanın amacı akut koroner sendromlu hastalarda CRP ve homosisteinin, çok damar hastalığı ile ilişkisi olup ol­ma­dığının saptanmasıdır. Gereç ve Yöntem: Çalışmaya akut koroner sendromlu 83 hasta alınmıştır. Hasta grubunda anjiografik olarak tespit e­dilen damar tutulum sayısı, yaygınlığı, stenoz şiddeti ve damar skoru ile homosistein ve CRP düzeyleri kar­şı­laş­tı­rıl­mış­tır.Damar skoru; (0-3 puan; damar tutulum sayısına göre), stenoz skoru; (0-3 puan: 0 puan: darlık yok, 1; %50’nin altında darlık, 2; %50-%75 arasında lezyon, 3; %75’ten fazla darlık), yayılım skoru; (0-3 puan: toplam lez­yon uzunluğunun segment tutulumuna göre yayılım). Bu 3 skorun toplamından, toplam skor bulunmuştur. 83 has­ta­nın 11’inde normal ya da minimal koroner arter hastalığı (Grup A: skor 0-3), 45’inde orta şiddette koroner arter has­ta­lığı (Grup B: skor 4-6), 27’sinde ciddi koroner arter hastalığı ( Grup C: skor 7-9) saptandı. İstatistiksel analiz için Krus­kal-Wallis yöntemi kullanıldı. Bulgular: Orta şiddette (Grup B) ve ağır şiddette (Grup C) koroner arter hastalığı olan bireylerde CRP seviyesi ha­fif şiddettekilere (Grup A) göre daha fazla bulunmuştur, ancak bu fark istatistiksel olarak anlamlı bulunmamıştır (Grup A: 5.33±3.71 mg/L, grup B: 8.41±11.40 mg/L, Grup C: 6.76±10.80 mg/L p>0.05). Orta şiddette (Grup B) ko­ro­ner arter hastalığı olan bireylerde homosistein seviyeleri hafif şiddette koroner arter hastalığı olanlara göre daha faz­la bulunmuştur (Group A: 14.89±4.12 mikromol /L, Group B: 16.97±6.21 mikromol/L, Group C: 14.82±4.16 mik­romol/L, p>0.05). Diğer taraftan, diabetik olmayan hastalarda orta şiddette (Grup B) ve ağır şiddette (Grup C) ko­roner arter hastalığı olanlarda CRP seviyesi koroner arter hastalığı hafif olan bireylere göre daha yüksek bulundu an­cak bu fark istatistiksel olarak anlamlı bulunmadı (Group A: 5.61±3.95 mg/L, Group B: 7.05±8.45 mg/L, Group C: 6.72±8.72 mg/L, p>0.05). Orta şiddette (Grup B) koroner arter hastalığı olan hastalarda homosistein seviyesi mi­ni­mal şiddetteki koroner arter hastalarına göre daha yüksek bulundu ancak Kruskal-Wallis yöntemiyle istatistiksel fark­lılık saptanmadı. (Group A: 17.38±4.99 mikromol /L, Group B: 18.40±6.95 mikromol/L, Group C: 15.32±4.77 mik­romol/L, p>0.05). Tüm bunlara ek olarak diabetik hastalarda ciddi koroner arter hastalığı ile homosistein ve/veya CRP seviyeleri arasında anlamlı bir ilişki saptanmadı. Sonuç: Sonuçta yapmış olduğumuz çalışmada, akut koroner sendromlu bireylerde diabetik olsun veya olmasın, CRP ve homosistein seviyesi ile çok damar hastalığı arasında istatistiksel olarak anlamlı bir ilişki saptanmamıştır.

Abstract

Aim: High homocysteine level and/or C-Reactive Protein (CRP) levels are not only related to atherosclerosis but al­so to the severity of coronary artery disease. In this study we aimed to determine the relation of multivessel co­ro­nary disease and homocysteine and CRP levels in acute coronary syndrome. Material and Method: We studied 83 patients who were referred for coronary angiography with acute coronary syn­dro­me. The relation between serum homocysteine and CRP levels and the severity and extension of coronary le­sions was studied. The coronary angiograms were evaluated according to three scores. Vessel score (0-3 points for 0-3 vessels with coronary artery disease), stenosis score (0-3 points; number and severity of coronary stenoses or lesions; 0 for no, 1 for coronary lesion with diameter stenosis less than %50, 2 for %50-%75 and 3 for more than %75 diameter stenosis), and extension score (0-3 points; segment-extension of all coronary lesions within the total co­ronary vessel length). Finally the total score values obtained from the vessel score, stenosis score, and extent sco­re. From the 83 patients, 11 were found to have no or minimal coronary artery disease (group A; score 0-3), 45 had moderate (group B; score 4-6) and 27 had severe (group C; score 7-9) coronary artery disease assessed by co­ronary angiography. For the statistical analysis Kruskal-Wallis test had been used. Results: In moderate(Group B) and severe groups of coronary artery disease, CRP levels were found higher than mild group but the difference was found to be insignificant with Kruskal-Wallis method (table 1) (Group A: 5.33±3.71 mg/L, Group B: 8.41±11.40 mg/L, Group C: 6.76±10.80 mg/L, p>0.05). In moderate(Group B) group of co­ronary artery disease, homocysteine levels were found higher than mild group but there was no statistically sig­ni­fi­cant differences with Kruskal-Wallis method. (Group A: 14.89±4.12 mikromol/L, Group B: 16.97±6.21 mikromol/L, Gro­up C: 14.82±4.16 mikromol/L, p>0.05). On the other hand, in nondiabetic patients (table 2); In moderate(Group B) and severe groups of coronary artery disease, CRP levels were found more than mild group but there was no sta­tistically significant differences with Kruskal-Wallis method. (Group A: 5.61±3.95 mg/L, Group B: 7.05±8.45 mg/L, Group C: 6.72±8.72 mg/L, p>0.05). In moderate(Group B) group of coronary artery disease, homocysteine le­vels were found higher than mild group but there was no statistically significant differences with Kruskal-Wallis met­hod. (Group A: 17.38±4.99 mikromol /L, Group B: 18.40±6.95 mikromol/L, Group C: 15.32±4.77 mikromol/L, p>0.05). In addition to these findings, there was no statistically significant relationship between severe CAD and ho­mocysteine and/or CRP levels in diabetic patients. Conclusion: Finally, using the scoring system in diabetic or nondiabetic patients, the relationship between the severity of multivessel coronary disease and homocysteine and/or CRP levels were statistically insignificant.
