Koroner Arter Ektazisi Olan Hastalarda Trombosit İndekslerinin Değerlendirilmesi

Özet
Amaç: Trombosit sayısı, plateletkrit, ortalama trombosit hacmi ve trombosit dağılım genişliği gibi trombosit indeksleri trombositlerin değişkenlik büyüklüğünü ölçmektedir. Koroner arter ektazisinin patogenezinde rol oynayabilirler. Bu çalışmada koroner arter ektazisi ve trombosit indeksleri arasındaki ilişkiyi araştırılmıştır. Gereç ve Yöntem: Bu retrospektif çalışmada, Ocak 2016 ile Aralık 2017 arasında koroner anjiyografi yapılan 72 hasta (37 erkek; ortalama yaş 59 ± 8 yıl), 65 yaş ve cinsiyet eşleştirilmiş kontrol grubu dahil edildi. Başlangıç özellikleri kaydedildi. Tüm deneklerin, tam kan sayımları dahil olmak üzere rutin kan testleri alındı. Bulgular: Kontrol grubu ile bazal özellikler açısından tek anlamlı fark, koroner arter ektazisi grubunda daha yüksek sıklıkta hipertansiyon olmasıydı (p=0,035). Koroner arter ektazisi olan hastalarda, ortalama trombosit hacmi (8,5 ± 0,9 ve 7,2 ± 1 mm, p 8,3 fL olması, %62 duyarlılık ve %68 özgüllük ile (EAA=0,795, p=0,002) ve plateletkrit >%0,2 olması %60 duyarlılık ve %65 özgüllük ile (EAA = 0,623, p=0,005) koroner arter ektazisi varlığını tahmin etmiştir. Sonuç: Ortalama trombosit hacmi ve plateletkrit gibi trombosit indekslerindeki artışlar, korner arter ektazisinin aterosklerotik süreci ile ilişkili olabilir
Abstract
Objective: Platelet indices such as platelet count, plateletcrit, mean platelet volume and platelet distribution width measure the variability size of thrombocytes. They may play a role in the pathogenesis of the coronary artery ectasia. We investigated the relationship between coronary artery ectasia and platelet indices in this study. Material and Method: This retrospective study included 72 patients (37 male; mean age, 59±8 years) with coronary artery ectasia and 65 age- and sex - matched controls, who underwent coronary angiography between January 2016 and December 2017. Baseline characteristics were recorded. All subjects had routine blood tests including common blood cell counts. Results: The only significant difference with the control group with respect to baseline features was the higher frequency of hypertension in the coronary artery ectasia group (p=0.035). Mean platelet volume (8.5±0.9 vs 7.2±1 mm, p 8.3 fL predicted the presence of coronary artery ectasia with a sensitivity of 62% and a specificity of 68% (AUC=0.795, p=0.002) and the plateletcrit level of >0.2% predicted the presence of AAD with a sensitivity of 60% and a specificity of 65% (AUC=0.623, p=0.005). Conclusion: The increases in platelet indices such as mean platelet volume and plateletcrit may be related to the atherosclerotic process of coronary artery ectasia.
