Ventrikül fonksiyonları bozuk olan hastalarda glukoz-insülin-potasyum solüsyonun kullanımı ve sonuçları

Özet
Amaç: Bu çalışmadaki amacımız ventrikül fonksiyonları bozuk olan (ejeksiyon fraksiyonu [EF] 2 mgr./dl), akut renal yetmezliği olan (idrar çıkışı 5.5 mmol/lt), karaciğer yetmezliği olan (total bilb >2.5 mgr/dl, SGOT ve SGPT>100 IU) hastalar çalışmaya dahil edilmedi.Diabetik olmayan hastalarda GIK solüsyonu; 500cc %30 dekstroz, 80mEq/L K ve 50 ünite kristalize insülin konularak hazırlandı. Diyabetik olanlarda ise GIK solüsyonu %10 dekstrozdan 500cc kullanarak hazırlandı. İnfüzyon anestezinin indüksiyonu ile beraber 1ml/kg/saat olacak şekilde başlandı ve aorta kross klemp konulduktan sonraki ilk 12 saat infüzyona aralıksız devam edildi. Hastaların peroperatif ve postoperatif dönemlerde kardiyak indeksleri, inotrop ihtiyaçları ve aritmi insidansları değerlendirildi. Bulgular: Bu çalışmada her iki grupta mortalite saptanmadı. GIK solüsyonu kullanılan grup ile kullanılmayan grubun postoperatif kardiyak indeksleri( CI)'leri arasında anlamlı fark saptanmadı (GIK kullanılan grupta postoperatif ortalama CI=2.49lt/m2/dk, GIK kullanılmayan grupta ortalama CI=2.37lt/m2/dk olarak saptandı (p 0.001). Sonuç: Literatürler ve elde ettiğimiz veriler eşliğinde ventrikül foksiyonları bozuk olan hastalarda peroperatif ve postoperatif dönemde GİK solusyonu verilmesinin ventrikül fonksiyonlarının desteklenmesi açısından iyi bir alternatif tedavi yöntemi olduğunu söyleyebiliriz.
Abstract
Aim: The aim of this study was to evaluate the effect of the glucose-insulin-potassium (GIK) solution on the myocardial function, postoperative arrythmia incidence and the length of hospitalization of the patients with depressed ventricular fuınction (EF<40%). Material and Method: We randomly seperated 60 patients with depressed ventricular functions into two groups. We also included the diabetic patients in the study. Patients with chronic renal failure (blood creatinine >2 mg/dl), with acute renal failure (urine output<20 ml/hr), with hyperkalemia (K level >5.5 mmol/lt), with hepatic failure (total bilirubine >2.5 mg/dl, SGOT and SGPT >100 IU) were excluded from the study. GIK solution was prepared as a mixture of 500 cc 30% dextrose, 80 mEq/L K and 50 units regular insulin for non-diabetic patients, whereas 500 cc of 10% Dextrose was used for the diabetic ones. Infusion was started on the induction of anesthesia as 1 ml/kgr and following the cross-clamping of the aorta it was constantly given for the first 12 hours. We measured the cardiac functions peroperatively and postoperatively, need for the inotropic medication and incidence of the arrythmias. Results: There was no deaths in both groups. The postoperative cardiac index of the GIK used group was significantly higher compared to the group that GIK wasn't used (p<0.001). Other parameters including the incidence of the atrial fibrillation and ventricular fibrillation, postoperative inotropic support,the length of the mechanical ventilation and the stay in the ICU and the hospital weren't statisticaly different in two groups (p>0.001). Conclusion: In the lights of the findings of our study, we may claim that the use of GIK solution in the peroperative and postoperative periods of the patients undergoing CABG with depressed left ventricular function is a favorable method.
