Kandesartan sileksetil kullanılan primer hipertansiyonlu hastalarda izlem çalışması

Özet

Amaç: Kandesartan sileksetil, hipertansiyon tedavisinde etkinliği ve güvenliliği kanıtlanmış bir anjiyotensin II tip 1 reseptör blokeridir. Bu çalışmanın amacı, kandesartan sileksetil kullanılan primer hipertansiyonlu hastalarda antihipertansif etkinlik ve istenmeyen olaylar ile ilgili klinik deneyimin belirlenmesidir. Gereç ve Yöntem: Bu araştırma ulusal, çok merkezli, karşılaştırmasız ve müdahelesiz bir çalışma olarak düzenlenmiştir. Çalışmaya Türkiye'de çeşitli merkezlerden 541 pratisyen hekim tarafından izlenen 18 yaş üzerinde, primer hipertansiyon nedeniyle tek başına kandesartan sileksetil kullanması uygun görülen hastalar dahil edilmiştir. Çalışmaya dahil edilen hastalara kandesartan sileksetil tedavisi 8 mg dozunda başlanmış ve 1 ay sonunda hastanın diyastolik kan basıncı 90 mmHg ve sistolik kan basıncı 140 mm Hg sınırının üzerinde tespit edilmişse, kandesartan dozu günde 16 mg'a çıkarılmıştır. Hastalar ilk vizit sonrası 2 aylık takip süresi boyunca ayda bir olmak üzere iki kez izlenmişlerdir. Çalışmanın primer sonuç değişkenleri; görülen advers olaylar ve sıklıkları, advers olayın ilaç ile olan ilişkisi, yapılan klinik müdahale (antihipertansif tedavi) ve klinik sonuç (normotansiyonun sağlanması), sekonder sonuç değişkenleri ise; doz arttırma gereği ortaya çıkan hastaların oranı ve ilacın ka n basıncına olan etkisi olarak belirlenmiştir. Bulgular: Çalışmaya tüm merkezlerden belirtilen kriterlere uygun toplam 2261 primer hipertansiyon tanısı olan hasta (yaş ortalaması 56.4±11.1 yıl) dahil edilmiştir. Çalışmaya dahil edilen hastalarda en sık rastlanan anormal biyokimyasal bulgular hiperkolesterolemi (%13.0), hipertrigliseridemi (%5.0) ve hiperglisemi (%6.7) olmuştur. Kandesartan sileksetil tedavisi öncesi ortalama 168.4±19.7 mm Hg olan sistolik kan basıncı ve 100.8±10.8 mm Hg olan diyastolik kan basıncı, tedavinin 1. ve 2. ayında anlamlı derecede azalmıştır (sırasıyla 141.4±15.7/87.0±9.0 ve 133.7±12.4/83.2±7.3 mmHg). Tedavinin ilk ayı tamamlandığında hastaların %28.4'ünde kandesartan sileksetil dozunun artırımı gerekmiştir. Tedavi süresince tedavi öncesine göre hastaların ortalama kalp atım hızı, solunum hızı, vücut ağırlığı ve vücut kütle indeksi anlamlı olarak azalmıştır. Toplam 2098 hasta advers olaylar yönünden değerlendirilmiş ve bu hastalardan 48'inde 59 advers olay gelişmiştir. Gelişen advers olaylardan 29'u olasılıkla ya da büyük olasılıkla çalışma ilacı ile ilişkili bulunmuştur. En sık bildirilen advers olaylar sırasıyla baş ağrısı, baş dönmesi, bulantı, halsizlik, hipertansif atak, nefes darlığı ve öksürüktür. Sonuç: Kandesartan sileksetilin geniş hasta topluluklarında primer hipertansiyon tedavisinde kullanılması ile yüksek antihipertansif etkinlik ve güvenlilik profili gözlenmiştir.

Abstract

Aim: Candesartan cilexetil is an angiotensin II type 1 receptor blocker with proven efficacy and safety for hypertension treatment. The aim of this study was to determine clinical experience regarding antihypertensive efficacy and adverse events in patients with primary hypertension receiving candesartan cilexetil. Material and Method: This study was designed as a national, multi-centered, non-comparative and non-interventional study. Patients over 18 years of age with primary hypertension who were prescribed candesartan cilexetil by 541 general practitioners working at various health centers in Turkey were enrolled in the study. The patients enrolled in the study were administered an initial dose of 8 mg candesartan cilexetil daily and if their diastolic and systolic blood pressure values were above 90 and 140 mg Hg, respectively, at the end of the first month, candesartan dose was doubled to be 16 mg a day. The patients were monitored twice with one month interval following the initial visit. Primary result variables of the study were established as follows: adverse events encountered and their frequencies, relationship between adverse events and the study drug, clinical interventions (antihypertensive treatment) and outcome (normotension). Secondary result variables were determined to be the ratio of patients who req uired increased doses, and the effects of the medicine on blood pressure. Results: A total of 2261 patients (mean age 56.4±11.1 years) diagnosed with primary hypertension, complying the defined criteria were enrolled in the study from all centers. The most common abnormal biochemical laboratory findings in enrolled patients were hypercholesterolemia (13.0%), hypertriglyceridemia (5.0%) and hyperglycemia (6.7%). The mean systolic blood pressure which was 168.4±19.7 mm Hg and diastolic blood pressure which was 100.8±10.8 mm Hg prior to candesartan cilexetil administration was observed to decrease significantly in the first and second month of treatment (141.4±15.7/87.0±9.0 and 133.7±12.4/83.2±7.3 mm Hg respectively). An increased candesartan cilexetil dose was required for 28.4% of the patients upon completion of the first month of treatment. The patients' mean heart rate, respiration rate, and body weight and body mass index decreased significantly during treatment. A total of 2098 patients were evaluated with respect to adverse events and 48 patients experienced a total of 59 adver se events. Of these, 29 seemed to be either "probably" or "most probably" were related to the drug of the study. The most commonly reported adverse effects, in order of prevalence, were headache, dizziness, nausea, weakness, hypertensive episode, dyspnea and coughing. Conclusions: High antihypertensive efficacy and tolerable safety profile were observed when candesartan cilexetil was administered in a large patient population for primary hypertension treatment.
