Subklinik hipotiroidide tiroid replasman tedavisinin arteryel sertlik üzerine etkisi

Özet

Amaç: Aterosklerozun erken dönemlerinde olduğu gibi, subklinik hipotiroidide de arteryel sertlik artmıştır. Biz, subklinik hipotiroidide, tiroit replasman tedavisinin, arteryel sertliği azaltıp azaltmadığını araştırmak istedik. Gereç ve Yöntem: Çalışmaya, yeni subklinik hipotiroidi tanılı (yaş ortalaması 39.34±9.6 yıl), 34’ü kadın toplam 41 hasta alındı. Micro Medical Pulse Trace 6.000 Module cihazı ile hastaların bazal Puls Wave Velosite (PWV), Stiffness İndeksi (Sİ) ve Refleksiyon İndeksi (Rİ) değerleri elde edildi. Tiroid replasman tedavisi ile ötiroidinin sağlanmasını izleyen 6. ayda, PWV, Sİ ve Rİ ölçümleri tekrarlandı. Bulgular: Tedavi öncesi ve sonrasında elde edilen veriler karşılaştırıldığında, tiroit replasman tedavisi sonrası, Sİ ve PWV’de istatistik- sel olarak anlamlı azalmalar tespit edildi (p<0,001). Rİ’de ise anlamlı değişiklik olmadı (p= 0,261). Sonuç: Subklinik hipotiroidi, tiroit replasman tedavisi ile başarılı bir şekilde tedavi edilirse, uzun dönemde arteryel sertlikte azalma oluşmaktadır.

Abstract

Aim: The arterial stiffness is known to be increased in subcinical hypothyroidism. Increase in arterial stiffness has been shown also in the early stages of atherosclerosis. We assessed if thyroid replacement therapy causes a reduction in arterial stiffness in subclinical hypothroidism or not. Material and Method: Fourty one patients (34 female, 7 male, mean age 39.3±9.6 years ) newly diagnosed as subclinical hypothy- roidism were enrolled in the study. Basal Pulse Wave Velocity (PWV), Stiffness Index (SI) and Reflection Index (RI) values of the pati- ents recorded by Micro Medical Pulse Trace 6.000 Module. All of these parameters recorded again at the sixth month of the patiens that became euthyroid with replacement therapy. Results: The data of the patients before and after therapy was compared. There was statistically sinificant reductions of SI and PWV values with thyroid replacement therapy (p<0.001). But no significant change was observed with RI parameter (p= 0.26). Conclusions: Arterial stiffness seems to be reduced in subclinical hypothyroid patients if they are successfully treated with thyroid re- placement therapy.
