Özet

Amaç: Bu çalışmanın amacı serum albümin seviyeleri ile dipper/non-dipper patern arasındaki ilişkiyi göstermektir. Gereç ve Yöntem: Aralık 2015 ve Ağustos 2016 tarihleri arasında toplam 245 adet yeni hipertansiyon hastası çalışmaya alınmıştır. Tüm hastalara 24-saat ambulatuvar kan basıncı takibi yapılmıştır. Veri analizleri SPSS 22.0 istatistik paket programı kullanılarak yapılmıştır. Bulgular: Hastalar dipper ve non-dipper kan basıncı paternine göre iki gruba ayrıldı. 91 hastada non-dipper, 154 hastada ise dipper pa-tern izlendi. Çok değişkenli regresyon analizi serum albümin, C-reaktif protein ve diyastolik disfonksiyon ile non-dipper patern arasında anlamlı ilişkili olduğunu gösterdi. Son olarak, non-dipper paterni olan hastalarda serum albümin limitini tespit etmek için ROC eğri ana-lizi kullanıldı. Non-dipper paterni tespit etmek için limit albümin seviyesi %83,7 duyarlılık ve %73,4 özgüllük ile 4,3 olarak tespit edildi.

Abstract

Objective: The aim of this study was to investigate the association between serum albumin levels in patients with a dipper versus a non-dipper pattern. Material and Method: Totally, newly diagnosed 245 hypertension patients were enrolled in the study between December 2015 and August 2016. The 24-h ambulatory blood pressure monitoring was performed in all patients. SPSS 22.0 statistical package program (SPSS Inc. Chicago, IL) was used to perform all data analyses. Results: The patients were divided into two groups according to the status of the dipper or non-dipper pattern. While 91 patients had a non-dipper pattern, 154 patients had the dipper pattern. Multivariate regression model analyses showed that albumin, C-reactive pro-tein and diastolic dysfunction were significantly associated with the non-dipper pattern. Finally, ROC curve analysis was performed to detect the cut off value of serum albumin levels in patients with the non-dipper pattern. The cut-off value of serum albumin to predict non-dipper pattern was 4.3, with a sensitivity of 83.7% and a specificity of 73.4%. Conclusion: This study showed that there was a relationship between serum albumin levels and the non-dipper pattern.
