Özet

Amaç: Bu çalışmanın amacı aç veya tok olmak durumunun temel elektrokardiyografi paremetreleri üzerinde olan etkisini ve bunun kli-nik yansımalarını incelemektir. Gereç ve Yöntem: Çalışmaya toplam 54 gönüllü (ortalama yaş 35, 27 erkek) dahil edildi. Katılımcılar 17 saat hiçbir katı ya da sıvı gıda almadan bekledikten sonraki açlık elektrokardiyografi kayıtları ve yemekten 2 saat sonraki tokluk elektrokardiyografi kayıtları alındı. Standart 12 derivasyonlu elektrokardiyografideki kalp hızı, PR intervali süresi, QRS süresi, QT segmenti, QTc değerleri, ST segmenti deği-şikliği ve T dalgası değişikliği ve aritmik bulgular yönünden elektrokardiyografiler karşılaştırıldı. Bulgular: Açlık ve tokluk elektrokardiyografilerinde kalp hızı ve QT süreleri arasında anlamlı fark izlendi. Katılımcıların açlık kalp hızları anlamlı olarak daha düşük saptandı (66,16 vs 74,85, p<001). Açlık elektrokardiyografilerinde QT mesafesi belirgin olarak uzun izlendi (390,05 vs 366,51, p<001). QTc, PR ve QRS sürelerinde anlamlı fark izlenmedi. Elektrokardiyografilerde ST segment veya T dalga değişi-mi izlenmedi. Atriyal fibrilasyon, supraventriküler veya ventriküler taşikardiler ya da ekstra sistol açısından fark saptanmadı. Sonuç: Açlık, QT mesafesi ve kalp hızı üzerinde önemli bir etkiye sahip olup, elektrokardiyografide kalp hızı ve QT süresi değerlendirilir-ken hastanın açlık durumu da göz önünde bulundurulması gereken önemli bir faktördür.

Abstract

Objective: The purpose of this study was to investigate the effect of the state of fasting or satiety on basic electrocardiography param-eters and its clinical manifestations. Material and Method: A total of 54 volunteers (mean age 35, 27 males) were included in the study. Electrocardiography recordings of the participants were performed after 17 hours of fasting with no liquid or solid food intake, and then, electrocardiography recordings were made from the same participants 2 hours after eating. The standard 12- derivation-electrocardiography recordings were compared in terms of heart rate, PR interval duration, QRS duration, QT segment, QTc values, ST segment changes, T wave changes and arrhyth-mic findings. Results: Significant differences in heart rate and QT durations were observed between the fasting and post- prandial electrocardiogra-phy recordings. The heart rates of participants were determined to be significantly lower on fasting recordings (66.16 vs 74.85, p<001). The QT intervals were observed to be markedly prolonged on fasting ECG recordings (390.05 vs 366.51, p<001). There were no signifi-cant differences among the QTc, PR and QRS durations. ST segment or T wave changes were not observed on the electrocardiographies. There were no differences determined in terms of atrial fibrillation, supraventricular or ventricular tachycardia or extra-systole. Conclusion: Fasting has an important effect on the QT interval and the heart rate; the fasting status of the patient should be kept in mind as an important factor while assessing the heart rate and the QT duration on electrocardiographies.
