Özet

Amaç: Operasyon sonrası atriyal fibrilasyon, artmış morbidite ve mortalite ile ilişkilidir. SYNTAX skoru II son zamanlarda geliştirilmiştir ve perkütan koroner girişim veya koroner arter baypas greftleme uygulanan hastalar için mortalite tahminini sağlamaktadır. Bu çalışmada, SYNTAX skoru II ile koroner arter baypas greftleme sonrası atriyal fibrilasyon gelişimi arasındaki ilişkiyi değerlendirmeyi amaçlanmıştır. Gereç ve Yöntem: Haziran 2013 ile Haziran 2017 arasında koroner arter baypas greft ameliyatı geçirmiş olan ardışık hastaların tıbbi kayıtları, ameliyat sonrası dönemde atriyal fibrilasyon gelişimi açısından retrospektif olarak incelendi. SYNTAX skoru, SYNTAX skoru II, klinik ve ekokardiyografik parametreler değerlendirildi. Bulgular: Operasyon sonrası atriyal fibrilasyon gelişimi için bağımsız değişkenler tanımlandı ve prediktif değerleri ölçüldü. Çalışma grubu 54'ü (%34) operasyon sonrası atriyal fibrilasyon gelişen 178 hastadan oluşuyordu. Operasyon sonrası atriyal fibrilasyon gelişimi için yaş, hipertansiyon, kronik obstrüktif akciğer hastalığı, diabetes mellitus, sol atriyal çap, kreatinin klirensi, SYNTAX skoru ve SYNTAX skoru II önemli değişkenler olarak tanımlandı. Bununla birlikte, lojistik regresyon analizinde sol atriyal çap (RO=2,482, %95 GA=1,627-4,058, p=0,005) ve SYNTAX skoru II (RO=3,758, %95 GA=2,637-5,371, p=0,005) operasyon sonrası atriyal fibrilasyon gelişimini öngören bağımsız değişkenler olarak bulundu. Sonuç: SYNTAX skoru II ve sol atriyal çap operasyon sonrası atriyal fibrilasyon gelişiminin bağımsız öngördürücüleri olarak bulundu. SYNTAX skoru II, hem perkütan koroner girişim/koroner arter baypas greft işlemlerindeki mortalite hem de operasyon sonrası atriyal fibrilasyon gelişimini tahmin etmede eşsiz bir skorlama sistemi olarak kullanılabilir.

Abstract

Objective: Postoperative atrial fibrillation is associated with increased morbidity and mortality. SYNTAX score II recently developed and provided the mortality rate prediction for patients undergoing percutaneous coronary intervention or coronary artery bypass graft surgery. This study aimed to evaluate the relationship between the SYNTAX score II and the development of atrial fibrillation after coronary artery bypass graft surgery. Material and Method: The medical records of consecutive patients, who underwent coronary artery bypass graft surgery from June 2013 to June 2017, were retrospectively reviewed for the development of atrial fibrillation in the postoperative period. SYNTAX score, SYNTAX score II, clinical and echocardiographic parameters were evaluated. Results: The independent variables for the development of postoperative atrial fibrillation were defined, and their predictive values were measured. The study group consisted of 178 patients, which 54 (34%) developed postoperative atrial fibrillation. Age, hypertension, chronic obstructive pulmonary disease, diabetes mellitus, left atrial diameter, creatinine clearance, SYNTAX score and SYNTAX score II were identified as important variables for the development of postoperative atrial fibrillation. However, in logistic regression analysis left atrial diameter (OR=2.482, 95% CI=1.627-4.058, p=0.005), and SYNTAX score II (OR=3.758, 95% CI=2.637-5.371, p=0.005) appeared as independent variables predicting the development of postoperative atrial fibrillation. Conclusion: The SYNTAX score II and left atrial diameter were independently associated with the development of postoperative atrial fibrillation. SYNTAX score II can be used as a unique scoring system in predicting both mortality in percutaneous coronary intervention / coronary artery bypass graft surgery procedures and development of postoperative atrial fibrillation.
