Normotansif, prehipertansif ve hipertansif hastalarda koroner lezyon sayısı ve koroner arter hastalığı şiddeti

Özet
Amaç: Çalışmamızda prehipertansif evredeki hipertansiyon hastalarında koroner damar hastalığı sıklığı ve şiddetinin diğer evrelerdeki hipertansif hastalar ile karşılaştırılması amaçlandı. Gereç ve Yöntem: Çalışmaya 2001-2002 yılları arasında merkezimize stabil koroner arter hastalığı ön tanısıyla başvuran, elektif koroner anjiyografi yapılan 136 olgu alındı. Değerlendirmede hastalar Birleşik Ulusal Komitenin VII. raporu (JNC VII) kriterlerine göre normal, prehipertansif, evre I ve evre II olarak sınıflandırıldı. Normotansif grupta 29 hasta, prehipertansif evrede 30 hasta, evre I hipertansif hastaların olduğu grupta 30 hasta ve evre II hipertansif hastaların olduğu grupta 47 hasta yer alıyordu. Koroner anjiyografi sonrası hastaların >%50 stenoza sahip major epikardiyal koroner arter sayısı ve toplam koroner arter skorları belirlendi. Koroner arter hastalığının sıklık ve şiddetini etkileyen faktörlerin tesbiti için tek ve çok değişkenli lojistik regresyon analizi kullanıldı. Bulgular: Hastalarda tutulan damar sayısına bakıldığında normotansif hastalarla karşılaştırıldığında prehipertansif evredeki hastalarda bir damar (%17.2, % 26.7), iki damar (%3.4, %26.7) hastalığı oranlarının daha yüksek olduğu görüldü. Ordinal lojistik regresyon analizi yapıldığında hipertansiyonun evresi ile hipertansiyonun süresinin [relatif risk 0.23 (%95 güven aralığı 0.15-0.35)] ve tutulan damar sayısının [relatif risk 0.73 (%95 güven aralığı 0.54-0.98)] anlamlı olduğu saptandı (p<0.001). Sonuç: Bu çalışmada, prehipertansif evredeki hastalarda, koroner anjiyografi ile saptanan aterosklerotik epikardiyal arter sayısının ve koroner arter skorunun normotansif hastalara göre daha yüksek oranlarda olduğu görüldü. Bu bulgu bize hipertansiyonun, koroner arter hastalığının etyolojisindeki rolünü göstermektedir. Prehipertansif evredeki hastalarda, evre I hipertansifler kadar koroner arter hastalığına rastlanmıştır.
Abstract
Aim: The aim of this study was to compare the incidence and severity of coronary artery disease between prehypertensives and other stages of hypertensive patients. Material and Methods: Between 2001-2002, 136 patients who underwent coronary angiography were enrolled to the study. According to the staging system in the Seventh Report of the Joint National Committee (JNC VII) on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure, patients were categorized as normal, prehypertensive stage, stage I hypertensive and stage II hypertensive. There were 29 patients in normotensive stage, 30 patients in prehypertensive stage, 30 patients in stage I hypertension and 47 patients in stage II hypertension. Each angiogram was classified as revealing either no coronary lesion with >50% luminal stenosis or as having one, two, or three major epicardial coronary arteries with >50% luminal obstructions. In nine discrete segments, representing the proximal and mid portions of the coronary arteries (left main stem: one segment, left anterior descending artery: two segments, circumflex artery: three segments, and right coronary artery: three segments), coronary lesions were evaluated according to the following grading scale: <10% stenosis: 0 points, 10% to 49%: 1 point, 50% to 74%: 2 points, 75% to 90%: 3 points, 90%: 4 points. Univariate and multivariate logistic regression analysis were used for the assesment of factors that affect the incidence and severity of coronary artery disease. Results: Our study demonstrated that one vessel (17.2%, 26.7%) and two vessel (3.4%, 26.7%) coronary artery disease were more common in prehypertensives than normotensive patients. Moderate and severe coronary artery disease was more frequent in prehypertensive patients compared to normotensive patients, Ordinal logistic regression analysis revealed that stage of hypertension correlated with duration of hypertension [odds ratio 0.23 (95% confidence interval 0.15-0.35)], number of diseased vessel [odds ratio 0.73 (95% confidence interval 0.54-0.98)] (p<0.001). Conclusion: In this study, in prehypertensive patients the number of atherosclerotic coronary arteries and coronary artery score was significantly higher compared to the normotensive patients. This finding show us the role of hypertension in coronary artery disease in prehypertensive patients, the coronary artery disease was prevalent as in the case of stage I hypertension.
