Elektron ışınlı bilgisayarlı tomografi ile saptanan aort kalsifikasyonunun koroner arter hastalığı yaygınlığı ve ciddiyeti ile ilişkisi

Özet

Amaç: Aort duvarında elektron ışınlı bilgisayarlı tomografi (EIBT) ile saptanan kalsifikasyon ile koroner kalsiyum ve koroner arter hastalığı (KAH) arasındaki ilişkinin sorgulanması amaçlandı. Gereç ve Yöntem: Göğüs ağrısı yakınması nedeniyle koroner anjiyografi yapılması planlanan ve aynı hafta içinde EIBT çekilen 61 hasta çalışmaya alındı. EIBT yapılarak koroner kalsiyum skoru ile birlikte çıkan ve inen aort kalsiyum skorları da belirlendi. Koroner anjiyografide saptanan koroner arter hastalığının yaygınlık ve ciddiyetini gösteren Gensini ve Negri skorları hesaplanarak EIBT bulguları ile aralarındaki ilişki sorgulandı. Bulgular: Çıkan ve inen aort kalsifikasyonu olanlarda olmayanlara göre koroner kalsiyum skorları anlamlı farklılık göstermekteydi (p<0.001). Çıkan aortta kalsifikasyon varlığı ile hasta damar sayısı, Gensini ve Negri skorları arasında anlamlı ilişki saptandı (sırasıyla, r=0.290, p=0.03; r=0.299, p=0.02 , r=0.384 p=0.003). Çıkan aort kalsifikasyon skoru ile de hasta damar sayısı, Gensini ve Negri skoru arasında zayıf ilişki bulundu (sırasıyla, r=0.373, p=0.005, r=0.388, p=0.003, r=0.387, p=0.003). İnen aort kalsiyum varlığı ile hasta damar sayısı, Gensini ve Negri yaygınlık skorları arasında ilişki bulunmazken inen aort kalsiyum skoru ile sadece Negri skoru arasında zayıf ilişki saptandı (r=0.327, p=0.01). Bu ilişki multivariate analizle hipertansiyon, yaş ve cinsiyet için düzeltildiğinde de devam etti (p=0.01). Sonuç: EIBT ile saptanan çıkan aort kalsifikasyonu, total kalsiyum skoru, KAH varlığı ve yaygınlığı ile ilişkilidir.

Abstract

Aim: The aim of the present study was to evaluate the relationship between degree of calcification of the aortic wall, the degree of coronary calcium detected by electron beam computed tomography (EBCT) and the coronary angiographic findings in patients with coronary artery disease. Material and Methods: EBCT was performed within 1 week in 61 patients who had undergone elective coronary angiography for the diagnosis of coronary artery disease. Coronary calcium scores and calcium scores of ascending and descending aorta were determined by EBCT. Coronary angiograms were scored by Gensini and Negri scores to evaluate the extent and severity of associated coronary artery disease. The relationship between EBCT and coronary angiographic scores was sought. Results: There were statistically significant differences for coronary calcium scores in patients who had calcification in the ascending and descending aorta compared to patients who had not (p<0.001). Presence of calcification in the ascending aorta was associated with the number of vessels with disease, Gensini and Negri scores (r=0.290, p=0.03; r =0.299, p=0.02, r=0.384 p=0.003 respectively). Also the calcium score of ascending aorta was associated with the number of vessels with disease, Gensini and Negri scores (r=0.373, p=0.005, r=0.388, p=0.003, r=0.387, p=0.003 respectively). No relationship was observed between the presence of calcium in descending aorta and the angiographic scores. The calcium score of descending aorta was associated with only Negri score (r=0.327, p=0.01). The relationship was still significant after multivariate analyses using hypertension, age and sex as covariates (p=0.01). Conclusion: Calcification of ascending aorta is associated with total coronary calcium score, presence of coronary artery disease and the angiographic extent of coronary artery disease. Presence of aortic calcification detected in EBCT which was performed for coronary calcium screening may be helpful in the diagnostic procedure.
