Akut inferior miyokard infarktüsünde resiprok değişikliklerin değerlendirilmesinde doku doppler ve koroner anjiyografi

Özet
Amaç: Akut miyokard infarktüsünde (Mİ) görülen resiprokal ST-segment depresyonunun bazı çalışmalarda iskemiye bağlı olduğu, bazı çalışmalarda elektriksel aktivite değişikliklerinden kaynaklandığı gösterilmiştir. Çalışmanın amacı doku Doppler ekokardiyografiyle (DDE) bölgesel miyokardiyal iskeminin bir göstergesi olarak miyokard fonksiyon bozukluğunu değerlendirmek ve anjiyografik bulgularla karşılaştırmaktır. Gereç ve Yöntem: Resiprok değişiklik saptanan 41 hasta (Grup 1), saptanmayan 10 hasta (Grup 2) çalışmaya alındı. Bulgular: Doku Doppler ekokardiyografiyle ölçülen anterior duvar bazal ve mid sistolik (Sm, cm/sn) hızı, erken (Em, cm/sn) ve geç (Am cm/sn) hızları, Em/Am, deselerasyon zamanı (Em, ms) ve bölgesel izovolumetrik gevşeme zamanı (İVGZ, ms) (anterior duvar bazal sırasıyla 5.6$\pm$1.3 karşı 5.7$\pm$1.2 P=0.89, 7.2$\pm$2.2 karşı 6.4$\pm$2.5 P=0.29, 6.1$\pm$1.7 karşı 5.9$\pm$1.2 P=0.68, 1.2$\pm$0.6 karşı 1.1$\pm$0.4 P=0.34, 105$\pm$35 karşı 106$\pm$23 P=0.97, 70$\pm$21 karşı 79$\pm$16 P=0.26, anterior duvar mid sırasıyla 4.7$\pm$1.1 karşı 5.0$\pm$1.6 P=0.49, 6.1$\pm$1.7 karşı 6.1$\pm$0.7 P=0.99, 5.0$\pm$1.7 karşı 5.2$\pm$1.4 P=0.83, 1.3$\pm$0.5 karşı 1.2$\pm$0.3 P=0.64, 106$\pm$34 karşı 86$\pm$40 P=0.11, 72$\pm$26 karşı 68$\pm$23 P=0.62) arasında fark yoktu. Sol ön inen arter (SOİA) lezyonu grup 1'de 12(%29) hastada grup 2'de 2(%20) hasta saptandı (P=0.43). Sonuç: Bulgular akut inferior Mİ'de erken dönemde görülen resiprok değişikliklerin elektriksel aktivite değişikliklerinden kaynaklandığını göstermektedir.
Abstract
Aim: It is suggested that reciprocal ST-segment depression in acute myocardial infarction to result from ischemia in some studies and from electrical activity changes in the others. The aim of the study was to assess myocardial dysfunction as an indication of regional myocardial ischemia using tissue Doppler echocardiography (TDE) in patients with reciprocal changes and to compared with angiographic findings. Material and Method: Reciprocal ST-segment depression was present in 41 patients (Group 1) but not in the remaining 10 patients (Group 2). Results: There were no significant differences between groups 1 and 2 with respect to systolic (Sm cm/sn) 5.6$\pm$1.3 vs 5.7$\pm$1.2 P=0.89, early (Em cm/sn) 7.2$\pm$2.2 vs 6.4$\pm$2.5 P=0.29, late (Am, cm/sn) 6.1$\pm$1.7 vs 5.9$\pm$1.2 P=0.68, diastolic waves peak velocities, E/A ratio 1.2$\pm$0.6 vs 1.1$\pm$0.4 P=0.34 Em wave deceleration time (DT, ms) 105$\pm$35 vs 106$\pm$23 P=0.97, regional isovolumetric relaxation time (IVRT, ms) 70$\pm$21 vs 79$\pm$16 P=0.26, and systolic (Sm) 4.7$\pm$1.1 vs 5.0$\pm$1.6 P=0.49, early (Em) 6.1$\pm$1.7 vs 6.1$\pm$0.7 P=0.99, late (Am) 5.0$\pm$1.7 vs 5.2$\pm$1.4 P=0.83, diastolic waves peak velocities, E/A ratio 1.3$\pm$0.5 vs 1.2$\pm$0.3 P=0.64, Em wave deceleration time (DT) 106$\pm$34 vs 86$\pm$40 P=0.11, regional isovolumetric relaxation time (IVRT) 72$\pm$26 vs 68$\pm$23 P=0.62) in anterior wall (basal and mid levels respectively) which correspond to reciprocal ST segment changes on electrocardiography. Left anterior descending artery stenosis was detected 12 (%29) patients in group 1 and 2 (%20) patients in group 2 (P=0.43). Conclusions: These findings suggest that reciprocal ST-segment depression during the early phases of inferior MI result from the changes in the electrical activity.
