Rilmenidin ve lisinoprilin sol ventrikül hipertrofisine etkisinin karşılaştırılması

Özet
Amaç: Antihipertansif ilaçların arteryel kan basıncını (AKB) düşürmeleri yanında hedef organ hasarlarını da önlemesi ve geriletmesi beklenir. Bu çalışmada lisinopril ve rilmenidinin sol ventrikül hipertrofisini geriletici etkileri karşılaştırılmıştır. Gereç ve Yöntem: Çalışmaya GATA Kardiyoloji Kliniğinde tekrarlanan 3 ölçümde sistolik kan basıncının³ 140 mmHg ve/veya diyastolik kan basıncının $\geq$90 mmHg olduğu orta-hafif hipertansiyonlu 38 hasta (ort yaş: 56±8) alındı. İlk 19 hastaya lisinopril, ikinci 19 hastaya rilmenidin başlandı. Sol ventrikül kitlesinin hesaplanmasında Penn formülü kullanıldı. Lisinopril grubundaki 4 hasta yan etki (3 öksürük; 1 allerjik reaksiyon) nedeniyle; rilmenidin grubundaki 3 hasta da tedaviye uyum göstermediklerinden çalışma dışında bırakıldı. Başlangıç ve üç ay sonraki sürekli değişkenler Student's t testi ile karşılaştırıldı. P değerinin 0.05). Sonuç: Sonuç olarak, rilmenidinle lisinopril arasında sol ventrikül hipertrofisini geriletme açısından fark olmadığı saptanmıştır.
Abstract
Aim: Antihypertensive agents are expected to regress and prevent end organ damage, besides their effect of lowering arterial blood pressure. In this study, the regressing effects of rilmenidine and lisinopril on left ventricular hypertrophy were compared. Material and Method: Thirty-eight patients (Mean age: 56±8) with mild to moderate hypertension in whom diagnosis of hypertension was accepted as systolic blood pressure³ 140 mmHg and/or diastolic blood pressure³ 90 mmHg on three repetitive measurements. Of these, first 19 patients were chosen to take lisinopril while the second ones rilmenidine. Continous variables in the beginning and the third month were compared with Student's t test. P<0.05 was considered significant. Results: Four patients of the lisinopril group because of the side effects such as cough and allergic reactions and 3 patients of the rilmenidine group because of maladjustment were excluded. So, the statistical analysis was performed in 15 patients of lisinopril group and 16 patients of the rilmenidine group. Mean blood pressure analyzed with ambulatory blood pressure monitoring decreased in both groups. The decrement was higher in the rilmenidine group. Interventricular septum, left ventricle posterior wall diastolic thickness and left ventricular mass showed statistically significant differences when compared at the beginning and the end of the treatment. Similar regression of left ventricular mass index was observed in both groups ( Rilmenidine group: from 187±34 gr/m2 to 179±31 gr/m2, lisinopril group: from 199±28 gr/m2 to 191±27 gr/m2; p>0.05) (Rilmenidine group 4.3% decrease vs lisinopril group 4% decrease, p>0.05). Conclusion: In conclusion, left ventricular hypertrophy regressed with rilmenidin in equivalent rate with lisinopril.
