Dual antiplatelet tedavi alan akut koroner sendromlu hastalarda proton pompa inhibitörü kullanımının majör kardiyovasküler olay gelişimi üzerine etkisi

Özet

Amaç: Akut koroner sendrom (AKS) tanısı olan hastalarda, aspirin ve klopidogrel günümüzde halen en temel tedavi seçeneği olup, du- al antiplatelet tedavi (DAPT)’nin major kardiyovasküler olayları azalttığı bilinmektedir. Buna karşın DAPT’nin ciddi gastrointestinal (GIS) yan etkilere neden olduğu gösterilmiştir ve bu yan etkileri önleyebilmek amacıyla pek çok hastada tedaviye proton pompa inhibitörleri (PPI) eklenmektedir. PPI ile beraber klopidogrelin kullanımının, major kardiyovasküler olayları artırıp arttırmadığına ait literatürde çe- lişkili sonuçlar bulunmaktadır. Bu çalışmada AKS tanısı ile yoğun bakımda takip edilen hastalarda, DAPT’ye ek olarak PPI kullanımının major kardiyovasküler olay gelişimi üzerine etkisini ve bu etkinin PPI’lar arasında farklı olup olmadığını araştırmayı planladık. Gereç ve Yöntem: Bu çalışmaya AKS tanısı ile yoğun bakıma yatmış olan 303 hasta alındı. Hastaların tamamı aspirin ve klopidogrel kullanmaktayken, 232 hasta PPI kullanmaktaydı. PPI kullananlar ise lansoprazol, pantoprazol, omeprazol ve rabeprazol kullananlar ol- mak üzere 4 gruba ayrıldılar. Bu hastalarda hastane içi angina pektoris, nefes darlığı, elektrokardiyografi(EKG) değişikliği, kardiyak be- lirteç yüksekliği, kalp yetersizliği(KY), aritmi ve reinfarkt gelişimi, koroner anjiyografi(KAG) ve perkütan koroner girişim(PKG) ihtiyacı, GIS kanama gelişimi ve mortalite araştırıldı. Aynı verilere taburculuk sonrası 1.ay ve 6. ay içinde bakıldı. Bulgular: Çalışmamızda PPI kullananlar ve kullanmayanlar arasında hastane içi angina pektoris, dispne, aritmi, KY ve AKS gelişimi, KAG veya PKG ihtiyacı, GIS kanama gelişimi veya mortalite açısından fark saptanmamıştır. PPI grupları kendi arasında değerlendiril- diğinde, omeprazol kullanan grupta nefes darlığı ve KY gelişimi daha fazla olarak görülmekle birlikte, başlangıçta da bu hastaların ne- fes darlığı ve KY ile başvurduğu veya intraaortik balon pompası ihtiyacı olup elimizde intravenöz PPI olarak tek seçenek olan omepra- zol başlanan hastalar olduğu tespit edildi. Hastalarda 1. ve 6.ay sonunda kardiyovasküler olay gelişimi açısından fark saptanmadı. Sonuç: Akut koroner sendrom tanısı ile yoğun bakımda takip edilip DAPT başlanan hastalarda PPI kullanımının, kardiyovasküler olay gelişimi üzerine olumsuz etkisinin olmadığı ve klopidogrel ile birlikte PPI tedavisi kombinasyonunun güvenli olduğu; PPI alt grupları arasında da fark olmadığı kanaatine varılmıştır.

Abstract

Aim: Aspirin and clopidogrel are still primary treatment options for patients with acute coronary syndrome (ACS). The application of dual antiplatelet therapy (DAPT) is also known to reduce major cardiovascular events for such patients. However, DAPT has been shown to have serious gastrointestinal (GI) side effects and to avoid these side effects the proton pump inhibitors (PPI) are added to the treatments for many patients. There are conflicting reports in the literature whether the use of PPI with clopidogrel increases major cardiovascular events or not. In this study, we planned to investigate the effects of the use of PPI, in addition to DAPT, towards developing major cardivascular events on the patients treated in the intensive care unit, and whether these effects differ among PPIs. Material and Metod: Three-hundred-three patients with ACS from the intensive care unit were included in this study. While all of the patients were using and clopidogrel, 232 of them were using PPI. The patients using PPI were divided into 4 groups according to the types of PPI which are lansoprazole, pantoprazole, omeprazole and rabeprazole. In-hospital angina pectoris, shortness of breath, changes in ECG, cardiac enzyme elevation, heart failure, arrhythmias, and reinfarct development, coronary angiography (CAG) and percutaneous coronary intervention (PCI) requirement, the development of GI bleeding and mortality were examined on these pa- tients. The same data was evaluated at first and sixth months after discharge. Results: In our study no difference was observed between PPI users and non-users in terms of in-hospital angina pectoris, dyspnea, arrhythmia, the development of heart failure and ACS, need for CAG or PCI, development of GI bleeding and mortality. When the PPI groups were evaluated among each other, the development of dyspnea and heart failure was seen more in the patients using omepra- zole. However, it was realized that either these patients had initally been accepted with the shortness of breath and heart failure com- plaints, or they had needed an intra-aortic balloon pump and had been treated with omeprazol as the only intravenous PPI option available to us at the time. At the end of the first and sixth months there were no differences found in terms of the development of cardiovascular events. Conclusion: It is concluded that the PPI use does not have any negative impacts of causing development of cardiovascular events in intensive care unit patients who are diagnosed with acute coronary syndrome and who are receiving DAPT. Moreover, it is seen that the combination of clopidogrel and PPI treatments are safe. In addition, no differences were observed among the subgroups of PPI.
