Hipertansiyon Hastalarında Tedaviye Uyum ve İlişkili Faktörler

Özet
Amaç: Hipertansiyon hastalarında tedaviye olan uyumun sağlanması ve sürdürülmesi, hedef kan basıncı değerlerine ulaşmak için önemli bir durumdur. Bu çalışma hipertansif hastaların tedavi rejimine olan uyumları ve ilişkili faktörlerin belirlenmesi amacıyla yapılmıştır. Gereç ve Yöntem: Veriler bir üniversite hastanesinin kardiyoloji kliniğinde farklı endikasyonlarla yatarak tedavi gören, en az 6 ay önce hipertansiyon tanısı konulmuş ve antihipertansif ilaç kullanan 234 hastadan toplantı. Verilerin toplanmasında Hill-Bone hipertansiyon tedavisine uyum ölçeği ve hipertansiyon bilgi düzeyi öiçeği kullanıldı. Bulgular: Hastaların Hill-Bone hipertansiyon tedavisine uyum ölçeğinden aldıkları puan ortalaması 8,15±5, hipertansiyon bilgi düzeyi ölçeğinden aldıkları puan ortalaması 14,24±6 olarak bulundu. Hipertansiyon tedavi rejimine uyum ölçeği puan ortalamalarının cinsiyet (p=0,004), eğitim durumu (p=0,001), ek bir kronik hastalık varlığı (p=0,034), kullandığı antihipertansif ilaç sayısı (p=0,005), evde kan basıncı takibi yapma durumu (p <0,001) ve hipertansiyona ilişkin bilgi düzeylerine (p <0,001) göre farklılık gösterdiği bulundu. Hastaların bilgi düzeyi ölçeği puan ortalamaları ile hipertansiyon tedavi rejimine uyum ölçeği puan ortalamaları arasında negatif yönde anlamlı ilişki bulundu (p <0,001). Sonuç: Çalışma hipertansiyon hastalarının tedavisine olan uyum düzeylerinin yetersiz olduğu ve hipertansiyona ilişkin bilgi düzeyinin artmasının tedavi uyumu arttırmada etkili olduğunu göstermiştir
Abstract
Objective: Ensuring and maintaining compliance with treatment in hypertension patients is an important step in achieving target blood pressure values. This study was carried out to determine the compliance of hypertensive patients with the treatment regimen and related factors. Material and Method: Data were collected from 234 patients admitted to a cardiology outpatient clinic of a university hospital, who were diagnosed with hypertension at least 6 months ago and who were taking antihypertensive drugs. Hypertension Knowledge - Level Scale and Hill-Bone Hypertension Treatment Compliance Scale were used to collect data. Results: Average score on hypertension treatment compliance scale was 8.15±5 and average score on hypertension knowledge level scale of patients was 14.24±6, hypertension treatment regimen scale mean scores was found to differ according to gender (p=0.004), educational status (p=0.001), presence of an additional chronic disease (p=0,034), number of antihypertensive drugs used (p=0.005), blood pressure monitoring at home (p <0.001) and hypertension to their knowledge level (p<0.001). There was a significant negative correlation between the hypertension knowledge level scale scores and hypertension treatment compliance scale scores of the patients (p <0.001). Conclusion: The study showed that the level of compliance with the treatment of patients with hypertension was insufficient and that the level of knowledge about hypertension was effective in increasing treatment compliance.
