Hipertansiyonlu Bireylerde Egzersizin Kan Basıncına Etkisi: Sistematik Derleme

Özet
Amaç: Bu sistematik derlemenin amacı, hipertansiyon tanısı almış bireylerde egzersizin kan basıncı üzerine etkisini değerlendirmektir. Gereç ve Yöntem: Sistematik derlemede, PRISMA-P (Preferred Reporting Items for Systematic review and Meta-Analysis Protocols) bil-dirgesi rehber alınmıştır. Araştırma stratejisinde altı elektronik veri tabanı kullanılmıştır: CINAHL, MEDLINE, Web of Science, Cochrane Library, PUBMED ve TUBİTAK Ulakbim Türk Tıp Dizini. Veri tabanları başlangıçtan 31/01/2016 tarihine kadar dil sınırlaması olmaksızın taranmıştır. Taramalarda “hypertension” and “exercise” and “blood pressure” MeSH terimleri ile “hipertansiyon” ve “egzersiz” ve “kan basıncı” anahtar kelimeleri kullanılmıştır. Bulgular: Derlemeye 1392 katılımcı ile 20 çalışma dahil edilmiştir. Hipertansif bireylerin örnek alındığı çalışmalarda, kan basıncını dü-şürmeye yönelik yapılan girişimlerin farklılık göstermesi ve bazı çalışmalarda örneklem sayısının az olması nedeniyle meta-analiz yapı-lamamıştır. Randomize kontrollü yapılan çalışmaların birçoğunda egzersizin kan basıncı üzerinde olumlu bir etkisinin olduğu bulunmuş-tur. Sonuç: Çalışmaların çoğu kan basıncını düşürmede egzersizin etkili bir faktör olduğunu; egzersiz ile birlikte kilo kontrolü ve diyet danış-manlığı (sağlıklı beslenme) gibi diğer yaşam tarzı değişikliklerinin kan basıncı kontrolünde daha etkili olduğunu göstermektedir. Sedanter yaşam tarzının benimsendiği toplumumuzda hipertansif hastalar ile çalışan sağlık profesyonelleri bireylere egzersiz programları ile kilo verme, diyet danışmanlığı gibi sağlıklı yaşam programları hazırlayabilir, bu programların uygulanmasında bireylere liderlik yapabilir.
Abstract
Objective: The purpose of this systematic review, individuals with a diagnosis of hypertension are to evaluate the effects of exercise on blood pressure. Material and Method: In this systematic review, declaration of the PRISMA-P (Preferred Reporting Items for Systematic review and Meta-Analysis Protocols) has been used as guidance. Six electronic databases were used in the research strategy: CINAHL, MEDLINE, Web of Science, Cochrane Library, PUBMED and TUBITAK Ulakbim Turkish Medical Index. Databases scanned without the language restriction from inception until 31 January 2016. In the scanning, “hypertension” and “exercise” and “blood pressure” MeSH terms with “hiper-tansiyon”, “egzersiz” and “kan basıncı” keywords were used. Results: Twenty studies with 1392 participants were included in the review. Meta-analysis was not possible due to showing the differ-ence of the interventions for reducing blood pressure and small sample size in some studies, in the studies of hypertensive individuals sampled. In the most of the randomized controlled trials, it was found that exercise was positive effect on the blood pressure. Conclusion: Most studies show that exercise is an effective factor in reducing blood pressure. Exercise along with other lifestyle changes such as weight control and diet counseling (healthy eating) is more effective in reducing blood pressure. In our society that have adopted a sedentary lifestyle, healthcare professionals who work with individuals with hypertensive patients can prepare healthy life programs such as exercise programs wtih weight loss, diet counseling to them and lead in the implementation of these programs.
