Stabil Koroner Arter Hastalarında Gensini Skoru ile Beck Depresyon Puanı Arasındaki İlişki

Özet

Amaç: Koroner arter hastalarında depresyon insidansı yaygındır ve olumsuz sonuçlara yol açmaktadır. Bu çalışmada stabil koroner arter hastalığı ile takip edilen hastalarda, Beck depresyon puanı ile Gensini skoru arasındaki ilişkiyi incelemek. Gereç ve Yöntem: Bu çalışmaya Eylül 2018 - Haziran 2019 tarihleri arasında stabil koroner arter hastalığı tanısıyla Pamukkale Üniver si - tesi Tıp Fakültesi Hastanesi Kardiyoloji Bölümü’nde takip edilen ve koroner anjiyografi yapılan 67 hasta dahil edildi. Koroner arter hastalığı yaygınlığı ve ciddiyeti Gensini skoru ile belirlendi. Hastalara koroner anjiyografi öncesi Beck depresyon ölçeği uygulandı. Hastalar depresyon ölçeğine göre gruplandırıldı (0-9 puan minimal depresyon, 10-16 puan hafif depresyon, 17-29 puan orta depresyon, 30-63 puan şiddetli depresyon). Bulgular: Çalışmaya dahil edilen hastaların yaş ortalaması 61,6 ± 9,9 yıldı ve %59,7’si erkekti. 50 hastada minimal depresyon, 11 hastada hafif depresyon ve 6 hastada orta depresyon saptandı. Üç grup arasında bazal demografik özellikler ve laboratuvar parametreleri açısından anlamlı fark yoktu. Kanada anjina sınıflamasına göre değerlendirildiğinde ortalama anjina skorları; minimal depresif bozukluk grubunda 2,48±1,09, hafif depresif bozukluk grubunda 2,36±0,809 iken orta depresif bozukluk grubunda 2,67±1,3 saptandı ve istatiksel fark yoktu (p=0,858). Grupların ortalama Gensini skorları; minimal depresif bozukluk grubunda 8,9±15,8, hafif depresif bozukluk grubunda 15±16,8 iken orta depresif bozukluk grubunda 7,7±8,2 saptandı. Gruplar arasında Gensini skoru açısından fark yoktu (p=0,481).

Abstract

Objective: The incidence of depression is common in coronary artery disease and leads to negative consequences. In this study, we aimed to investigate the relationship between the Beck depression scale and Gensini score in patients with stable coronary artery disease. Material and Method: This study included 67 patients who were followed up at the Cardiology Department of Pamukkale University Medical Faculty between September 2018 and June 2019 and underwent coronary angiography with the diagnosis of stable coronary artery disease. The prevalence and severity of coronary artery disease was determined by Gensini score. Beck depression scale was applied before coronary angiography. The patients were grouped according to the depression scale (0-9 points for minimal depression, 10- 16 points for mild depression, 17-29 points for moderate depression, 30-63 points for severe depression). Results: The mean age of the study population was 61.6±9.9 years and 59.7% of them were men. 50 patients had minimal depression, 11 patients had mild depression and 6 patients had moderate depression. There was not any significant difference in terms of basal demographic characteristics and laboarotary paramaters between groups. When evaluated according to Canadian angina classification, mean angina scores were; 2.48±1.09 in the minimal depressive disorder group, 2.36±0.80 in the mild depressive disorder group, and 2.67±1.30 in the moderate depressive disorder group, and there was no statistical difference (p=0.858). The mean Gensini scores of the groups were; the mean depressive disorder group was 8.9±15.8, the mild depressive disorder group was 15±16.8, and the moderate depressive disorder group was 7.7±8.2. There was no difference between the groups in terms of Gensini score (p=0.481).
