Keratokonuslu olgularda HLA antijen özellikleri

Özet
Amaç: Türk toplumunda keratokonus ile HLA (Klas-1) antijenleri arasındaki ilişkinin incelenmesi. Gereç ve Yöntem: Çalışmaya aralarında akrabalık bulunmayan 37 keratokonuslu hasta dahil edildi. Kontrol grubu olarak da normal sınırlarda göz bulguları olan 100 kişi alındı. HLA sınıflaması mikrolenfositotoksisite yöntemi kullanılarak gerçekleştirildi ve elde edilen sonuçlar Fischer doğrulama testi ile değerlendirildi. Bulgular: Keratokonuslu hastalarda kontrol grubu ile kıyaslandığında HLA -A3, -B44, -Cw4 ve -Cwx antijenleri sıklığında anlamlı bir artış gözlenirken (p=0.0185; p=0.0052; p=0.0115; p=0.0405 ), HLA -57, -557, -Bw4 ve -Cw antijenleri sıklığında anlamlı bir azalma saptandı (p=0.0367; p=0.0105; p=0.0296; p=0.0030). Keratokonuslu olgularda ayrıca HLA -A29, -A3O, -A33, -A34, -A36, -A43, -A66, -A69, -A74, -B13, -527, -B37, -B41, -550, -554, -55(5, -557, -B64, -570, -Bwx, -CwI, -Cw2 ve -Cw7 antijenlerinin bulunmadığı görüldü. Sonuç: Keratokonus ile HLA -A3, -B44, -Cw4 ve -Cwx gibi bazı HLA antijenleri arasında, toplumda bu hastalığın ortaya çıkmasını kolaylaştıracak bir ilişkinin varlığından söz edilebilir.
Abstract
Purpose: To investigate the association ofHLA class !antigens with keratoconus in Turkish population. Material and Methods: Thirty seven consecutive unrelated patients with keratoconus were enrolled into the study. One hundred healthy subjects with normal ophthalmological findings served as controls. HLA typing was performed in all participants using microlymphocytotoxicity method and the results were analyzed using Fischer's exact test. Results: A significant increase of HLA -A3, -B44, -Cw4 and Cwx antigens (p=0.0185; p=0.0052; p=0.0115; p=0.0405 respectively) and a significant decrease of HLA - 57, -557, -Bw4 and -Cw antigens (p=0.0367; p=0.0105; p=0.0296; p=0.0030 respectively) were detected in keratoconus patients when compared to the controls. Complete absence of HLA -A29, -A30, -A33, -A34, -A36, -A43, -A66, -A69, -A74, -Bl3, -527, -B37, -B41, -550, -554, -555, -557, -B64, -570, -Bwx, -CwI, -Cw2 and-Cw7 antigens was also noted among this group. Conclusion: There may be an association between keratoconus and some HLA class I antigens such as HLA -A3, -B44, -Cw4 and -Cwx antigens which may predispose eyes to develope this corneal disorder in Turkish population.
