Evaluation of high Dk fluorosilicone hydrogel contact lenses used on a 30-day continuous wear schedule

Özet

Amaç: Yüksek Dk'lı Florosilikon Hidrojel kontakt lenslerin, 30 gün süre ile aylık devamlı kullanımının klinik değerlendirilmesi ve güvenilirliğinin ortaya koymak. Gereç ve Yöntem: Florosilikon hidrojel kontakt lenslerle, (Focus Night & Day, Novartis) 30 gün aralıksız gece aşırı lens kullanma programını uygulayan 127 hastanın, 248 gözü, en az 12 ay, en çok 21 ay (ort. 14.4±2.9) boyunca izlendi. Hasta grubu; yas ortalaması 27.6 (±7.9) olup, 38 erkek (%29.1) ve 89 (%70.1) kadından oluşuyordu. Hastaların 92'si (%72.4) daha önceden kontakt lens kullanan hastalardı. Seçilen yarıçap 226 gözde (%91.1) 8.6 ve 22 gözde (%8.9) 8.4 olup, izlem süresinde 6 hastanın 12 gözünde 8.6 dan 8.4 eğrilik yarıçapına geçildi. Bulgular: İzlem süresince 37 gözde (%14.9) çeşitli semptom ve bulgulara, 27 gözde de (%10.9) komplikasyonlara rastlandı. Bulber hiperemi 28 gözde (%11.3), limbal hiperemi 19 gözde (%7.7), limbal vaskülarizasyon 6 gözde (%2.4), korneal boyanma 10 gözde (%4.0), Müsin topları 20 gözde (%8.1) tanımlandı. Epitel mikrokisti ise hiç görülmedi. Karşılaşılan komplikasyonlar; Kontakt lense bağlı periferal ülser, 7 hastanın 9 gözünde (%3.6), Kontakt lense bağlı akut kırmızı göz, 4 hastanın 4 gözünde (%1.6), Allerjik konjonktivit 9 hastanın 18 gözünde (%7.3) (bu hastalardan 3 hastanın 6 gözü dev popüler konjonktivit) idi. Ev süreçte hiçbir hastada korneal enfeksiyona rastlanmadı. Daha önceden başka bir lens kullanan 92 hastanın 76'sı (%82.6) takip süresinde yeni lensleriyle daha iyi, 9'u (%9.8) farksız, 7'si (%7.6) daha kötü konfora sahip olduklarını bildirdiler. Bu hastalardan önceki dönemde limbal vaskülarizasyonu gelişmiş olan 25 hastadan (50 göz), 19' unda (38 göz) (%76.0) yeni dönemde bu sorunun kaybolduğu saptandı. Sonuç: Korneal uyumluma parametrelerine dikltat edilmesi kaydıyla florosilikon hidrojel kontakt lenslerin 30 günlük devamlı kullanımı güvenilir bulunmuştur.

Abstract

Purpose: Clinical evaluation and demonstration of reliability of high Dk fluorosilicone hydrogel contact lenses used on a 30-day continuous wear basis. Material and Method: Two hundred and forty eight eyes of 127 patients who applied fluorosilicone hydrogel contact lenses (Focus Night & Day, Novartis) used on a 30-day continuous wear schedule were followed up minimum 12 months and maximum 21 months (mean 14.4±2.9). Mean age ofthepatients was 27.6±7.9 and of these 38 (29.1%) were male and 89 (70.1%) were female. Ninty-two (72.4%) patients have worn other contact lenses before. Base curve of the contact lens used was 8.6 for 226 eyes (91.1%) and 8.4 for 22 eyes (8.9%). During follow-up base curve -was changed from 8.6 to 8.4 for 12 eyes of 6 patients. Results: Various symptoms and findings were seen in 37 (14.9%) eyes and complications in 27 (10.9%) eyes during follow-up period. Bulbar hyperemia in 28 (11.3%) eyes, limbal hyperemia in 19 (7.7%) eyes, limbal vascularization in 6 (2.4%) eyes, corneal staining in 10 (4.0%) eyes, mucin balls in 20 (8.1%) eyes were defined findings. No epithelial cyst was determined. Complications were as follows; contact lens induced peripheral ulcers (CLPU) in 9 (3.6%) eyes of 7 patients, Contact lens induced acute red eye (CLARE) in 4 (1.6%) eyes of 4 patients, allergic conjunctivitis in 18 (7.3%) eyes of 9 patients (giant papillary conjunctivitis was seen in 6 eyes of 3 patients). Corneal infection was not seen at any patient during this period. Seventy six (82.6%) of the 92 patients who have worn another contact lens before, told a better comfort, 9 (9.8%) no change and 7 (7.6%) worse comfort with the new contact lenses during follow-up period. Limbal vascularization, which was seen in 25 patients (50 eyes) before, was disappeared in 19 patients (38 eyes) (76.0%) at the new period. Conclusion: Usage of fluorosilicone hydrogel contact lenses on a 30-day continuous wear schedule is reliable if corneal fitting parameters are taken care.
