Özet

Sinostotik plagiosefali esas olarak unilateral koronal sütürün erken kapanması sonucu gelişir. Etkilenen tarafta orbita tavanının kısalması ile superior oblik hipofonksiyonu ve inferior oblik hiperfonksiyonu ve bazen karşı tarafa tortikollis gelişir. Sinostotik plagiosefalisi olan bir olgumuzda stabismus cerrahisi ile vertikal kayma düzeltildiği halde baş pozisyonu devam etti. Tortikollisin kayan gözün kapatılması ile düzelmesi boyundaki restriksiyona bağlı olmadığını gösteriyordu. Bu tip olgularda, derin bir anormal retinal korrespondans nedeniyle oküler tortikollisin düzelmeyebileceği düşünülmelidir.

Abstract

Synostotic Plagiocephaly Causing Superior Oblique Deficiency and Ocular Torticollis Synostotic plagiocephaly, which is basically caused by premature closure of unilateral coronal suture, leads to sagittal shortening of the orbital roof on the affected side. As a consequence of its shortness, an underaction of the superior oblique muscle and an overaction of the inferior oblique muscle occur on the affected side and sometimes there is a torticollis to the side opposite the coronal synostosis. In our patient with synostotic plagiocephaly the head posture persisted, though strabismus surgery improved the vertical deviation. As the torticollis disappeared on occlusion of the deviated eye, the head tilt was not due to the restriction of the neck muscles. The fact that a deep-rooted anomalous retinal correspondence may cause the persistence of ocular torticollis should be considerd in these cases.
