Özet

AMAÇ: Laser in situ keratomileusis (LASİK) sonrası, diffüz lamellar keratit (DLK) insidansımızı belirlemek, tabloya neden olan predispozan faktörleri tartışmak ve etkilenen olgularda görsel prognozu değerlendirmek için çalışmamızı düzenledik. GEREÇ VE YÖNTEM: Kliniğimizde miyopi, miyopik astigmatizma ve hipermetropileri nedeniyle LASIK tedavisi uygulanan 656 hastanın 1220 gözü retrospektif olarak değerlendirildi. Hastaların, biomikroskopik verileri ve görme keskinlikleri DLK gelişme sıklığı açısından incelendi. Ortalama yaşı 26.4±5.2 olan, 424 erkek ve 232 kadın hastadan oluşan çalışma grubunda tüm LASIK girişimleri 8.5 mm çaplı Hansatome mikrokeratome (Chiron Vision, Claremont, California) ve SVS Summit Apex Plus (Summit Tech Waltham, Mass.) 193 nm excimer laser kulanılarak gerçekleştirildi. BULGULAR: Diffüz Lamellar Keratit , on sekiz hastanın yirmialtı (%2.16) gözünde belirlendi. Kadın/Erkek oranı 10/8 idi. Operasyon sonrası ilk gün ara yüzeyde multifokal infiltrasyonların saptandığı 19 göz(%73) evre I DLK tanısı aldı. İlk bir hafta içinde ağrı, fotofobi ve sulanma şikayetleri ile belirlenen 6 göz(%24) ise evre II DLK olarak tanımlandı. Evre I ve II klinik seyir izlenen gözlerin hepsinde ortalama 10 günlük topikal %0,1 lik florometholone tedavisi sonrası, infiltrasyonlar tamamen geriledi. Olgulardan sadece birinde, evre III DLK ve stromal erimeye bağlı düzensiz astigmatizma gelişti. Bu hastada LASİK öncesi düzeltilmiş görme keskinliğinde üç sıra kayıp gelişti. Sekiz olguda klinik tablo bilateral gelişti. Evre I semptomları ile tanımlanan iki gözde DLK sekonder girişimde, iki gözde de epitelial ingrowth sonrası ara yüzey temizliği ardından saptandı. SONUÇ: LASİK sonrası gelişen DLK erken tanınmaması ve gerekli önlemler alınmaması halinde ciddi komplikasyonlara neden olabilecek bir tablodur.

Abstract

PURPOSE: To study the incidence, predisposing factors and visual outcomes of patients with diffuse lamellar keratitis (DLK) who underwent laser in situ keratomileusis in our clinic. MATERIAL AND METHODS: The biomicroscopic data and final visual outcome of 1220 eyes of 656 cases that had treated with LASİK for their myopia, myopic astigmatism and hyperopia were reviewed for DLK. Mean age of the 424 male and 232 female patients that took place in this study was 26.4±5.2 years. Hansatome microkeratome with a standard 8.5 mm radius (Chiron Vision. Claremont. California) and 193 nm excimer laser (SVS Apex Plus Summit Tech.Waltham, Mass.) used in all procedures. RESULTS: Diffuse lamellar keratitis was seen in 26 eyes of 18 patients (2.16%). Female / Male ratio was 10/8. 73%( 19 eyes) of the cases that had presented at the first day after the surgery with the multifocal infiltrates located at the interface defined as stage I DLK, 24 % of the cases (6 eyes) who had been noticed in the first week with pain photophobia and tearing accepted as stage II DLK. All eyes with stage I and II symptoms were treated with topical 0,1% florometholone and infiltrates resolved. One eye had lost three lines of best spectacle-corrected visual acuity. This was the only case that was defined as stage III and complicated with stromal melting. Symptoms were bilateral in eight cases Two eyes who were presented with stage I DLK symptoms, were recognized after the second cut. These were cases that had undergone LASIK treatment for the second time because of the undercorrection. The other two cases had appeared with mild DLK symptoms after the enhancement therapy. CONCLUSION: DLK is a potentially serious complication of LASIK unless it is diagnosed early.
