Behçet hastalığında HLA B 51-52 ile oküler komplikasyonların ilişkisi ve prognoz yönünden önemi

Özet
AMAÇ: Behçet hastalığının etiyolojisi tam bilinmemekle birlikte otoimmün mekanizma defekti, herpes virüsları ve streptokoklar patogenezden sorumlu tutulurlar. Bu çalışmada Behçet hastalığının HLA-B 51-52 varlığında immünogenetik predispozisyonunu araştırmak ve görme prognozu ile olan ilişkisini değerlendirmek amaçlanmıştır. GEREÇ VE YÖNTEM: Otuzdört Behçet hastası çalışma kapsamına alındı. Olgular oküler komplikasyon varlığı yönünden iki gruba ayrıldılar. Grup I'de oküler komplikasyon saptanan 24 olgu,Grup II'de oküler komplikasyon saptanmayan 10 olgu mevcuttu. Hasta gruplarından elde edilen sonuçlar 100 sağlıklı bireyden oluşan kontrol grubu ile karşılaştırıldı. BULGULAR: Ortalama takip süresi 14 ay olan Grup I'de HLA-B 51-52, %83.3 (20/24), takip süresi 36 ay olan Grup II'de HLA-B 51-52 oranı %60 (6/10) olarak bulundu. Kontrol grubunda bu oran %30 (30/100) idi. Grup I ve II arasındaki fark istatiksel olarak anlamlı bulunmadı (P=0.15). Grup I ile kontrol grubu arasındaki fark ise çok anlamlı idi (P 0.05). Oküler komplikasyon olan grupta ise HLA-B 51-52 ile atak sıklığı, cinsiyet, görme keskinliğinde azalma, üveitin ciddi seyri ile istatiksel bir ilişki bulunmadı (sırasıyla P=0.33, P=0.36, P=0.33, P=0.56). Yine de bu grupta son kontrolde görme azalmasının %57.1 olarak bulunması daha geniş çalışmaların farklı sonuçlar oluşturabileceğini düşündürmektedir. SONUÇ: Bu çalışma HLA-B 51-52'nin Behçet hastalığının immünogenetik etiyolojisinde sorumlu faktörlerden biri olabileceğini fakat klinik bulguların özellikle göze ait komplikasyonların gelişiminde etken olmadığını, bu konuda multifaktöriyel mekanizmaların göz önünde bulundurulması gerektiğini göstermektedir
Abstract
PURPOSE: Although the etiology of Behçet's disease was not well understood, autoimmune mechanisms, herpes viruses and streptococci were considered responsible for the pathogenesis of the disease. The aim of this study was to investigate the effect of HLA-B 51-52 as an immunogenetic predisposing factor for Behçet's disease and its relationship with the visual prognosis. MATERIAL AND METHODS: Thirty four patients with Behçet's disease were included in this study. Patients were divided into two subgroups according to the presence of ocular complications. Group I included 24 patients with ocular complications whereas group II included 10 patients with no ocular complications. The results obtained from patient groups were compared with those obtained from a control group including 100 healthy control subjects. RESULTS: HLA-B 51-52 positivity was 83.3% (20/24) in Group I for a mean follow-up period of 14 months and 60% (6/10) in Group II for a follow-up period of 36 months. This positivity was 30% (30/100) in the control group. The difference between Group I and Group II was not statistically significant (P=0.15). The difference between Group 1 and the control group was statistically significant (P<0.001). There was no difference among the groups in terms of age, sex and the recurrence rate of the Behçet's disease (P>0.05). In Group 1, there was no correlation between HLA-B 51-52 positivity and the recurrence rate, sex, decrease in visual acuity or the severity of uveitis (P=0.33, P=0.36, P=0.33, P=0.56 respectively). However, different results may be observed in larger series as the decrease in visual acuity was 57.1% at the last visit in Group 1. CONCLUSIONS: This study shows that HLA-B 51-52 may be an immunogenetic factor in the etiology of Behçet's disease. However, it is not involved in the development of clinical findings, especially the ocular complications. Multifactorial mechanisms should be considered in this aspect.
